Job Description

We are recruiting for a full time certified Peer Support Specialist for our Assertive Community Treatment Team. This highly skilled Interdisciplinary Team serves Vance, Granville, Franklin and Warren Counties. Services are provided for 50 community based clients with Serious and Persistent Mental Illness, based on an evidenced based model of care. Primary Purpose of the Position This position provides hands on WRAP Training and interaction with clients to maintain stabilization in the Community. Work Schedule Monday - Friday 8:00 am - 5pm Individual programming/activities Maintains a work environment that supports dignity and fair treatment Carry out assigned training sessions and activities with individuals (individual or group) in order to meet their needs as identified by the team and Person Centered Plan Participate in and be responsible for activities in habilitation training Carry out specific technical procedures in programs, professionally initiated and planned such as behavior programs, etc. Collect data and document status of treatment plans and goals on the WELLSKY platform Attend daily team meetings for specific individuals and assist with the formulation of the Individualized Program Plan or Service plan and evaluation of the individual's progress. Attend interim meetings to share input when program changes are necessary. Interact with individuals on an on-going basis to appropriately offer stimulation and emotional support. Ensure that appropriate routine and/or required documentation is completed (i.e. Progress notes, incident reports, etc.) Attend staff meetings, staff planning sessions giving input into individual and group home/area needs. Ensure all Legacy Human Services, Inc. Policies and Procedures are carried out consistently. Immediately inform management of concerns or issues. Ensure adherence to TMACT standards, Desk Audit standards and CARF standards Other duties as assigned Follow instructions provided to employee. The employee receives written and verbal instructions from the Director of Legacy Human Services, Inc. and follows them. Follows guides, regulations, policies and references given to the employee. Legacy Human Services, Inc. Policies and Procedures, G. S. 122-C, Client Rights, USDA, Medicaid Policies and Procedures and certain homes also are held to HUD Policies and Procedure Standards. Supervision Received by Employee. Employees usually work without on-site supervision. Supervision is provided through monthly individual supervision sessions via QP Supervision plans, and otherwise as needed. Variety and purpose of personal contacts. Parents and guardians visit residents from time to time and employee(s) must be cordial and helpful to these individuals. The employee must interact well with service delivery team members from the community, various agencies and community Qualifications The Peer Support Specialist must have and maintain the Peer Support Specialist certification prior to hire.
