Brooke Army Medical Center Fort Sam Houton TX Respiratory Therapy Burn Center Day Shift 6:45a-3:15p, Evening Shift2:45p-11:15p, Night Shift 11:45p-7:15a, 7A-7P 6:45-7:15p, 7P-7A 6:45p-7:15a. Some overtime or call requirement. The Respiratory Therapy Burn Center will establish a duty schedule 30 days prior to the upcoming month. Starting and ending times as well as the specific days of the week to be worked will vary according to the unit's needs. The contractor shall provide Respiratory Therapy personnel to support a seven (7) day a week, 24-hour a day operation to include holidays. Below is an example of the most common shift times which consist of eight (8) work hours with a non-paid 30-minute lunch or dinner break, and a 12-hour shift with a 30-minute non-paid lunch or dinner break. The maximum number of shift hours to be scheduled in a twenty-four period is twelve, unless an emergency situation occurs. Duties Select and assemble proper respiratory equipment, supplies, and appropriate therapeutic agents for each service to be performed. Comprehensive patient assessment, including medical history, physical examination, and diagnostic data review. Continuous monitoring of respiratory status using auscultation, percussion, observation, vital signs, pulse oximetry, and capnography. Interpretation of arterial blood gas (ABG) results and recommendation of appropriate interventions. Collection of arterial blood samples for blood analysis by means of arterial punctures and patient arterial line sampling to meet College of American Pathologists standards. Processing blood samples utilizing Point of Care testing Arterial Blood Gas (ABG) machines located in clinical patient care areas. Proactive identification and management of adverse respiratory events. Accurate and timely documentation of all assessments, interventions, and patient responses in the MHS Genesis Electronic System. Management of artificial airways (endotracheal tubes, tracheostomy tubes, LMAs), including insertion, maintenance, suctioning, and troubleshooting. Administration and discontinuation of mechanical ventilation using conventional and advanced modes of ventilation, including high-frequency oscillatory ventilation. Administration of inhalation therapy involving nebulized beta-agonist, antibiotics, and saline, administered via metered-dose inhalers or via handheld nebulizer units. Administration and discontinuation of oxygen therapy appliances, including nasal cannulas, face masks/tents, venture masks, T-piece set-ups, Continuous Positive Airway Pressure (CPAP) devices, and oxygen tents. Performance of chest physiotherapy and endotracheal suctioning in adult, pediatric, and neonatal patients. Administration of manual ventilation via anesthesia-type bags, both during cardiopulmonary resuscitation emergencies and during routine suctioning. Collection arterial blood samples for blood analysis by means of arterial punctures and patient arterial line sampling to meet College of American Pathologists standards. Processing blood samples utilizing Point of Care testing Arterial Blood Gas (ABG) machines located in clinical patient care areas. Patient instruction regarding incentive spirometry. Set up and monitoring of inhaled nitric oxide systems. Initiation, operation, and monitoring of state-of-the-art respiratory life support systems and laboratory equipment unique to the Certified Respiratory Therapy Services. Calibration of complex respiratory equipment, identification, and correction of mechanical malfunctions to unique respiratory therapy equipment. Use of state-of-the-art transcutaneous monitoring for the evaluation of adequate ventilation and perfusion. Response to and management of emergent airway situations. Collaboration with physicians, nurses, and other healthcare professionals to provide optimal patient care. Effective communication with patients, families, and the healthcare team. Participation in departmental meetings and committees. Assistance with training of new respiratory therapists and other healthcare professionals. Provide training to active-duty RT students in Respiratory Therapy principles, procedures, techniques, equipment, procedural and processes, as well as hands-on training for Quality Assurance/Quality Control (QA/QC) purposes.
Burn Center:
The contract therapist will develop, implement and maintain continued performance/quality improvement activities within the section of clinical assignment. Establishes and evaluates experimental/new approaches to direct patient intervention and care-based in formation available in professional literature. Adapts current concepts and develops new ones to achieve established and professionally recognizable goals for patient care. Exercise professional judgement in planning and administering therapy to patients with inhalation, thermal, electrical, chemical or trauma injuries and associated other symptoms or other diseases that would place the patient in the care of the USAISR Burn Unit. Complete burn and trauma in-patient and outpatient evaluations appropriate for the patient to determine current levels of function/dysfunction. Assist in developing and conduct professional short courses and in-service education programs for peers, other health care providers and members of the military and civilian communities. Directly participates in all patient treatment. Evaluates and documents each patient response to treatment on an ongoing basis. Provides training to military personnel/ RT students in Respiratory Therapy principles, procedures, techniques, equipment, procedural and processes, as well as hands-on training for Quality Assurance / Quality Control (QA/QC) purposes. Design treatment plans to be implemented personally by the therapist or technicians assigned to United States Army Institute of Surgical Research (USAISR) Burn Unit. The contract therapist will develop, implement and maintain continued performance/quality improvement activities within the section of clinical assignment. Communicate findings to appropriate ISR staff. Evaluates and documents each patient's response to treatment on an ongoing basis. Initiates, operates and monitors stat of the art respiratory life support systems and laboratory equipment unique to the Burn Respiratory Therapy Section. Calibrates complex respiratory equipment and identifies and corrects mechanical malfunctions to unique burn respiratory therapy equipment. Use state of the arm transcutaneous monitoring for the evaluation of adequate ventilation and perfusion. Collects arterial blood samples for blood analysis by means of arterial punctures and patient arterial line sampling to meet College of American Pathologists standards. Processing blood samples within the Respiratory Therapy ABG lab and interpreting the results. Use independent clinical judgment, under the supervision of a physician to make appropriate ventilator and Bilevel Positive Airway Pressure (BiPAP)/Continuous Positive Airway Pressure (CPAP) settings changes based on the result of blood gas analysis within RT driven protocols. Recommend changes in treatment to the physician based on blood gas analysis. Provides pulmonary therapy by administering inhaled aerosolized medications, performing chest percussive therapy (CPT), lung expansion therapy, positive expiratory therapy (PEP), volumetric pulmonary exercises suctioning, bedside spirometry, oxygen level analysis and high flow oxygen therapy using specialized equipment. Performs intra-hospital patient transport for intubated burn Intensive Care Unit (ICU) patient radiological studies, procedures and wound care showers. Assist in inter-hospital patient movement needs for critical patients requiring extracorporeal membrane oxygenation (ECMO) support. Initiate respiratory therapy directed weaning protocols. Calibrates and initiate the use of specialty equipment and specialty gas therapy. Provide ventilator management for patients requiring support from the volumetric percussive resuscitator (VDR)-4, a highly specialized high frequency ventilator unique to the Institute of Surgical Research. Interfaces inhaled Nitric Oxide and Heliox adjuncts to patient life support systems with physician orders. Sets up and assists with therapeutic and diagnostic fiberoptic bronchoscopy procedures. Assist in the evaluation of inhalation injury and the collection of BAL samples for cytological and histological evaluation. Calibrates and performs Dual-Energy X-Ray Absorptiometry (DEXA) on burn ICU patients. Calibrates and performs metabolic studies using specialized equipment on burn ICU patients. Assists in the placement of artificial airways. Independently perform extubations, tracheostomy decannulation and tracheostomy tube exchanges with a physician's order. Provide primary assistance in cardiopulmonary resuscitation and serves as a member of the Rapid Response Team (RRT). Provides patient/family/staff education on treatment procedures. Makes timely and appropriate referrals to other providers when medical indicated. Progresses treatment as appropriate and demonstrates ongoing discharge planning. Therapists will attend weekly multidisciplinary discharge planning meetings as needed. Ensures correlation, coordination and communication among staff and health care members involved in promoting successful goal-oriented patient care. The contract respiratory therapist will maintain proper medical records on all patients evaluated and treated under this contract in accordance with AR 4066. The contract physical therapist will prepare any additional records or description of care if and when required on behalf of the MTF, the Medical Command, the Military Health System, Department of the Army and the Department of Defense. The contract therapist will be required to go through a six-week orientation (annual re-evaluation also), and will show proficiency in the following areas before being allowed to function without supervision: Suctioning (open and closed); nasotracheal and orotracheal suctioning, tracheostomy tube changes, mechanical ventilation both conventional and high frequency, using: VDR-4, Drager, Evita XL, transport ventilator (TXP), Duotron, long term ventilation (LTV),Sero-I, 3100 Oscillator and Oxylogy; aerosol therapy; bronchoscope/ gastroscope setup and procedures; CPT; incentive spirometry (IS); (Intrapulmonary Percussive Ventilation) IPV therapy; 02 therapy; high flow oxygen therapy; lung expansion therapy; patient transporters; arterial punctures; venipunctures; patient sample line blood draws; ABG analysis; pulmonary function test (PFTs); weaning parameters; ventilator changes outs; BiPAP/CPAP therapy; inhaled nitric oxide (INO) therapy, Heliox therapy; leaning and preparing equipment for sterilization; Essentris charting; culturing equipment; end-tidal carbon dioxide (CO2) monitors use, transcutaneous CO2 monitoring, arterial blood gas (ABG) lab maintenance and procedures; oxygen (02) cylinder use flow duration; infection control policies, Vocera system communications; metabolic studies and monitor use; DEXA scan bone densitometer, calibration and use. The contract therapist will perform daily shift duties and audits to include breaking down the workload, completion of daily quality checks, conducting chart reviews, and ensuring proper equipment disinfection and set up at shift's end to facilitate efficient workflow and maintain operational standards.

Degree/Education:
Associates Degree. Graduate from a program accredited by the Commission on Accreditation for Respiratory Care (CoARC) or Commission on Accreditation of Allied Health Education Programs (CAAHEP). Can also, be a graduate of a military Pulmonary Respiratory Therapist program. Must have completed an approved Respiratory Therapy educational program accredited by the National Board of Respiratory Care (NBRC), another approved organization or its predecessors. Certifications (in addition to Basic Life Support Possess certification as Certified Respiratory Therapist (CRT) by the National Board for Respiratory Care (NBRC). Must maintain current and active Certification throughout the life of the contract. Possess Advanced Life Support (ALS) from the American Heart Association (AHA) or American Red Cross (ARC), Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS), Neonatal Resuscitation Program (NRP) certification preferred at time of application however, if not already obtained must have NRP certification within three (3) months of on-boarding.

Must have a minimum of one (1) year of experience within the last two (2) years providing services as a Pulmonary Respiratory Therapist Familiar with adult, pediatric and Level III Neonatal Intensive Care Unit (NICU) neonatal practices working with premature infants.
