Job Description
Job Overview We are seeking a dedicated and compassionate Certified Home Health Aide (CHHA) to provide exceptional patient care in home and community settings. The ideal candidate will possess a strong background in senior care, assisted living, and working with individuals with disabilities or memory impairments. As a CHHA, you will play a vital role in supporting patients' daily needs, ensuring their safety, comfort, and well-being while adhering to all healthcare regulations and standards. This position offers an opportunity to make a meaningful difference in the lives of those requiring personalized care. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts. Monitor and record vital signs accurately, including blood pressure, pulse, temperature, and respiration rates. Administer medications following prescribed instructions while maintaining detailed medical documentation in Electronic Health Record (EHR) systems. Provide specialized dementia and Alzheimer's care, including memory care techniques to ensure patient safety and comfort. Assist with catheter care, wound management, and hospice support as needed to promote health and dignity. Support patients with medical procedures such as medication administration, catheter management, and hospice-related tasks. Perform light housekeeping duties such as cleaning, laundry, meal preparation, and ensuring a safe environment for patients. Maintain strict compliance with HIPAA regulations to protect patient confidentiality at all times. Qualifications Valid certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA) with current licensure. Prior experience in nursing homes, assisted living facilities, or home health environments is highly desirable. Demonstrated ability to work effectively with individuals with developmental disabilities or dementia-related conditions. Knowledge of medical terminology and familiarity with EMR/EHR systems for accurate documentation. CPR certification is required; additional training in hospice care or Alzheimer's/dementia care is a plus. Strong patient observation skills combined with compassionate caregiving abilities. Experience working with long-term care residents or in home & community care settings preferred. Ability to work independently while collaborating effectively within a multidisciplinary team environment.
