Pay Range:
$29.71

  • We are seeking a detail-oriented LPN Care Manager to support our clinical team in coordinating patient care.
Working under the direction of an RN Case Manager, you will help manage medical cases to ensure patients receive high-quality care in the most appropriate setting while promoting cost-effective outcomes.
Key Responsibilities Care Coordination:
Support hospital admission certification, continued stay reviews, and discharge planning.
Utilization Management:
Gather clinical information from the care team to assist in utilization reviews, following established policies and protocols.
Medical Review:
Conduct medical record reviews to support case management functions and ensure clinical accuracy.
Efficiency:
Oversee the delivery of healthcare services to ensure they are comprehensive, high-quality, and cost-effective.
Team Collaboration:
Work closely with the RN Case Manager and the broader care team to facilitate smooth patient transitions.
Qualifications & Requirements Licensure:
Current New York State LPN License.
Experience:
2-5 years of clinical experience in an acute care or applicable healthcare setting.
Clinical Knowledge:
Strong understanding of medical necessity and discharge planning processes.
Skills & Attributes Analytical Thinking:
Ability to review clinical data and apply utilization management protocols effectively.
Communication:
Strong interpersonal skills to interact with the care team and medical staff.
Organization:
Ability to manage multiple tasks and maintain accuracy in a fast-paced environment.
Professionalism:
A commitment to quality patient care and professional ethics.
