Chronic Care Coordinator (CCM Program)

Southwest Urgent Care Position Overview Southwest Urgent Care is seeking a highly motivated and patient-focused Chronic Care Coordinator to support our Chronic Care Management (CCM) program. This role is responsible for identifying eligible patients, conducting outreach, enrolling patients into the CCM program, and maintaining consistent communication to ensure continuity of care and improved health outcomes. This position plays a critical role in enhancing patient engagement, supporting preventative care initiatives, and driving program growth. Key Responsibilities Conduct outbound calls to eligible patients to introduce and enroll them in the CCM program Educate patients on the benefits of chronic care management services Maintain consistent follow-up communication with enrolled patients on a monthly basis Document all patient interactions accurately in the EMR system (eClinicalWorks or similar) Coordinate care plans and communicate relevant updates to providers and clinical staff Monitor patient eligibility and ensure compliance with CCM billing requirements Assist in managing patient care plans, including medication adherence and appointment scheduling Track and report performance metrics including enrollments, engagement rates, and retention Ensure all services meet CMS guidelines and compliance standards Qualifications Previous experience in healthcare, care coordination, call center, or patient outreach preferred Familiarity with Chronic Care Management (CCM) or Medicare programs is a strong plus Experience with EMR systems (preferably eCW) Strong communication and interpersonal skills Ability to build rapport and trust with patients over the phone Highly organized with strong attention to detail Self-motivated with the ability to meet performance targets Compensation & Benefits Competitive base salary + performance-based incentives (optional if you want to include bonuses) Opportunities for growth within a multi-location healthcare organization Training and development in care management program Why Join Us At Southwest Urgent Care, we are committed to improving patient outcomes through innovative care models and personalized attention.
