WHAT'S YOUR DAY LIKE?
As a Full-Time Chronic Care Nurse at North Mississippi Primary Health Care, your day-to-day responsibilities will include conducting comprehensive assessments of patients with chronic conditions, developing personalized care plans, and monitoring patient progress. You will collaborate with interdisciplinary teams to deliver coordinated care and provide education to patients and their families about managing their health.
