(Boston, MA and Chelsea, MA) (Multiple positions available): Under the supervision of the Program Director and Assistant Program Director, the Clinical Coordinator will participate in mental health assessments, treatment planning, and delivery of mental health services. discern peoples needs for rehabilitative services. Review and revise treatment plans. Help individuals improve their health and wellness; achieve symptom stabilization and self-management; develop and/or restore skills impacted by mental illness; and live self-directed lives. Guide individuals with psychiatric and substance use disorders to a life in recovery, and identify these peoples personal priorities, preferences, strengths, and interests. Engage in positive interactions that promote recovery and empowerment. Provide clinical services in timely, flexible ways that are consistent with the principles and practices of trauma-informed care and person-centered approaches. Participate in the development and implementation of safety plans and crisis prevention and response plans. Utilize screening and assessment tools. Utilize evidence-based practices including motivational interviewing, SBIRT, harm reduction, Stages of Change, Housing First, and others. Aid individuals in making informed choices that further recovery. Help individuals attain independence in their treatment and overall lives. Advocate for better access to public services and natural supports, facilitate peoples recovery and full integration into the community. Via confidential records and per established methods and procedures, document each persons treatment progress. Carry out the assigned interventions and action steps of treatment plans on individual, group, and family bases (in the office and in community settings). In GLE settings, provide interventions to assigned persons. As required by Medicaid Rehabilitation Option, document the need for rehabilitative services. Complete the mental status section of assessments. Alongside Integrated Team staff, develop, review, sign, and date treatment plans and all subsequent reviews and revisions. Participate in providing on-call services (including on evenings and weekends) per the on-call schedule. Participate in all required trainings. Adherence to all safety and risk mitigation policies and protocols. Participate in data gathering for outcome reporting. Prepare case summaries in preparation for clinical and risk reviews. Basic annual pay ranges from \$80,000 to \$93,000.
Minimum requirement:
Masters degree in Mental Health Counseling, Clinical Mental Health Counseling, Mental Health Counseling and Behavioral Medicine, Social Work, or a closely related field.
