Role Summary The Clinical Intake Specialist
  • LPN, under the supervision of a Registered Nurse (RN), serves as the first point of contact for individuals with complex care needs.
In this role, you'll conduct comprehensive in-person assessments, develop individualized plans of care, and provide ongoing support to ensure continuity and quality of services. You'll also play an essential part in educating participants and families about available programs and coordinating services that promote independence and safety. Key Responsibilities Conduct initial in-person visits and perform detailed health and needs assessments. Complete quarterly, annual, and as-needed reassessments following significant changes in condition. Develop and update individualized, person-centered Plans of Care and Service Plans. Provide options counseling to individuals and families to identify cost-effective and appropriate services. Collaborate with Care Managers, healthcare professionals, and community resources to coordinate care. Maintain accurate documentation in electronic health records (EHR) according to program standards. Identify and report potential critical incidents, safety concerns, or signs of abuse or neglect. Participate in interdisciplinary team (IDT) meetings and ensure compliance with all regulatory and organizational requirements. Required Qualifications Associate degree in Nursing or completion of a state-approved Practical Nursing program. Current and valid Licensed Practical Nurse (LPN) license. Minimum of 3 years of nursing experience, preferably in intake, triage, or assessment roles. Experience in community health, home health, or case management preferred. Knowledge of Medicaid, Medicare, and waiver programs preferred. Strong assessment, communication, and documentation skills.
