In this vital role, you will provide expert pharmaceutical care, ensuring safe and effective medication use across various hospital departments. Your expertise will support patient outcomes through comprehensive medication management, clinical assessments, and collaboration with multidisciplinary teams. Duties Conduct thorough patient assessments to determine medication needs, considering physiology, anatomy, and medical history. Collaborate with physicians, nurses, and other healthcare professionals to develop and optimize individualized medication therapy plans. Manage medication administration processes, including IV infusion preparation and aseptic technique adherence, ensuring safety and accuracy. Provide specialized care in critical care settings such as
ICU, PICU
(Pediatric Intensive Care Unit), and Level I/II trauma centers, addressing complex cases with precision. Oversee acute pain management and emergency room medication protocols to deliver timely relief and optimal patient outcomes. Monitor patient responses to medications, adjusting therapy as needed to maximize efficacy while minimizing adverse effects. Maintain comprehensive documentation of patient care activities, medication records, and compliance with hospital policies and medical terminology standards. Skills Extensive knowledge of pharmacy practices within managed care environments and hospital settings. Proven experience in Level II trauma centers, Level I trauma centers, ICU, PICU, and emergency room environments. Strong background in clinical pharmacy with post-residency experience preferred; including hospital medicine and critical care expertise. Proficiency in physiology, anatomy, pharmacology principles, and medical terminology relevant to acute care scenarios. Skilled in medication administration techniques such as IV infusion preparation and aseptic procedures. Experience managing pain medications and participating in acute pain management protocols. Ability to perform detailed patient assessments focusing on patient service and patient care excellence. Familiarity with managed care strategies to optimize medication therapy outcomes across diverse populations. Excellent communication skills for effective collaboration within multidisciplinary teams and educating patients on medication use.
