Category Pharmacy - Clinical Pharmacist Job Type Contract A healthcare facility in Carbondale, CO is seeking a skilled Clinical Pharmacist for a contract assignment beginning ASAP, with a projected end date of 8/15/26. This role involves working a full 40-hour week on a rotating shift schedule, including day and evening shifts Monday through Friday, plus rotating weekends from 7:00 AM to 12:00 PM.
Schedule:
Rotating shifts starting at either 6:30 AM or 8:00 AM, Monday to Friday Rotating weekends, 7:00 AM to 12:00PM Key Responsibilities:
Prepare and dispense medication orders accurately as per physician requests Manage medication stock in patient care areas to ensure availability Ensure safe, appropriate, and cost-effective medication therapies in accordance with policies and protocols Interpret and transcribe medication orders into computerized patient profiles Compound and dispense pharmaceuticals, including IV admixtures, chemotherapy, and parenteral nutrition Comply with all federal, state, and local regulations, managing controlled substances documentation Monitor drug therapy for contraindications, interactions, and appropriateness Perform pharmacokinetic drug dosing as per protocols Review culture and sensitivity data to guide antibiotic therapy Provide clinical consultation and drug information, suggesting therapeutic alternatives as needed Participate in medication review, quality improvement activities, and staff education programs Serve as a member of theCode Response Team Qualifications:
Active Colorado Pharmacist License in good standing Graduate of an ACPE-accredited School of Pharmacy with a BS Pharmacy degree BLS and ACLS certification from the American Heart Association Experience with Epic electronic medical records is required; Meditech experience is a plus Flu vaccine required (no exemptions) during flu season (Oct 1 - Apr 1) This assignment offers a unique opportunity to contribute during an EMR conversion from Meditech to Epic.
