About Square Medical Group (SMG) Square Medical Group's ("Square" or "SMG") mission is to deliver enduring, material improvements in both clinical and wellness outcomes to patients addressing co-occurring challenges with substance use disorder (SUD) and behavioral health (BH). We do this through delivering a specialized continuum of outpatient care of 1,000+ units/week to a population of ~5,000 across
Massachusetts:
medication management, in-person and virtual therapy for individuals, couples, and families, in-person and virtual intensive outpatient and partial hospitalization programs (IOP/PHP), transcranial magnetic stimulation (TMS), in-house laboratory services, as well as residential services. The residential recovery programs are structured rehabilitation-oriented treatment focused on trauma-informed care and harm reduction. It is designed for motivated individuals who want to enhance the quality of their day-to-day lives. Through active engagement in groups and the development of the community, we will work together to overcome challenges that interfere with our abilities to live life to the fullest. Residents will be assigned a weekly life skill to ensure ongoing cleanliness/maintenance of the house. Residents are expected to take part in assigned life skills. Staff will help if residents are unsure about how to complete a task. Life Skills are necessary to independent living. On average, residents spend between three to six months in the three phases of the residential program. Each phase of the program rewards residents with increasing levels of responsibilities and freedoms. Throughout the program, residents are supported by deep expertise from SMG's outpatient services, our core capability. Residents are also able to stay with our outpatient practice post-discharge for continuity of care.Role Summary:
Reporting directly to the house Program Director (PD) for all programmatic issues and to the Chief Medical Officer (CMO) for clinical issues, the CS is the clinical leader of the COERRS residence ensuring that all regulatory and organizational policies and procedures are being maintained and followed. The level of care integrates clinical excellence with a trauma-informed, person-centered environment, enabling residents to develop greater self-responsibility, stability, and life skills. Through structured programming and individualized care plans, the residence supports residents in preparing for long-term reintegration into the community whether through permanent housing, sober living, or other supportive pathways. As Clinical Supervisor, you will oversee the delivery of evidence-based clinical services and care coordination for residents working toward recovery from both moderate to severe mental health conditions and substance use disorders. You will coordinate clinical services with a multidisciplinary team to ensure that each resident receives goal-driven, outcome-oriented treatment aligned with SMG's mission of addressing social determinants of health and supporting lifelong recovery. This leadership role is pivotal in shaping a safe, accountable, and empowering therapeutic community where residents are equipped to reenter society with dignity, stability, and hope.Key Responsibilities:
1. Performance Metrics Monitor and report on key operational metrics, including: i.Bed Occupancy :
Collaborate with PD and Screener to achieve full occupancy at the residence through timely screening, onboarding, and discharge of residents ii.Staffing Levels :
Maintain a complement of clinical team members required for safe and effective care iii.Regulatory Compliance :
Ensure clinical adherence to all licensing and accreditation standards in coordination with the Compliance and Billing teams 2. Program Leadership a. Foster collaboration between operational and clinical priorities b. Weekly clinical meetings with the CMO c. Weekly supervisory meetings (i.e. 1-on-1s) with PD d. Provide coverage on-site at residences who have leaders out of office simultaneously under unforeseen circumstances (i.e. FMLA, termination, and resignation) e. Participate in on-call rotation. 3. Clinical Leadership a. Deliver evidence-based care tailored to residents' co-occurring disorders including individual & group counseling and crisis intervention/management b. Maintain daily group curriculum and ensure residents meet goals associated with groups and outside meetings c. Meet regularly with Nurse Practitioners (NP), Licensed Practical Nurses (LPN), clinicians, case managers, Floor Staff, and other residence staff as necessary to promote client needs d. Act in accordance with SMG's Standards of Care at all times, including maintaining professional ethics, boundaries and client confidentiality while working from a strength-based perspective using clinical approaches such as Motivational Interviewing, Cognitive Behavioral Therapy, Dialectical-Behavioral Therapy, Harm Reduction, Trauma Informed Care, and Self-Help Programming. 4. People Leadership (e.g., Staffing and Recruitment) Support recruitment, training, and retention strategies to ensure full clinical staffing in coordination with the PD; provide technical interviews for clinical positions, as requested Oversee onboarding on clinical staff (i.e. clinicians, case managers, and interns) and professional development programs to build a skilled, multidisciplinary clinical team Provide weekly clinical supervision for clinicians, case managers, and interns (as assigned) Completing necessary payroll and staffing related duties.Specific Tasks:
1. Oversee daily clinical activities of the COERRS program a. Deliver individual, group and crisis intervention therapeutic services b. Participate in on-call rotation as administrative back-up c. Review and approve program policies and procedures d. Assists all staff in becoming proficient in evidence-based practices (EBP) and providing fidelity to the EBP models 2. Resident-Centered Care a. Coordinate the admissions process Deliver therapeutic services to meet resident needs effectively Address and resolve resident concerns or issues escalated from clinical teams Promote a therapeutic and supportive environment that facilitates recovery and community reintegration 3. Administrative Excellence a. Review treatment planning and other clinical documentation, such as assessments/diagnosis, treatment plans, referrals, e.g., psychiatric and medication, aftercare/discharge and follow-up service plans b. Ensure clinical documentation, treatment plans, and services align with regulatory and organizational standards c. Perform client record reviews, including weekly MDT audits of open charts and a monthly review of closed charts d. Escalate clinical or resident care issues as needed e. Work independently and with the SMG billing team to ensure proper insurance authorizations are obtained on each resident in order to ensure 100% of utilized beds are paid for each day Required Skills forSuccess:
This position has an emphasis on organization, prioritization, effective communication (written and oral) and the ability to adhere to timelines. This individual will play a critical role in growth at SMG given the investment in development of the COERRS program and its associated real estate. This is a leadership-facing role with a need for a strong of teamwork and accountability. LADC I, or other independently licensed individual who has at least a master's degree in one of the following disciplines or a closely related field: Clinical psychology, education-counseling, medicine, mental health, psychology, psychiatric nursing, rehabilitative counseling, social work Two (2) years of supervised substance use disorder counseling experience One (1) year clinical supervisory experience Experience with EMR (InSync experience a plus) to maintain effective documentation and record keeping including biopsychosocial assessments, individual action plans, re-assessment, progress notes, case management, consent forms, data collection An advanced knowledge of state and federal regulatory and various accreditation requirements related to behavioral health management Experience with third-party insurance authorizations, quality and concurrent reviews and utilization management Experience working with BSAS and DPH a plus Other requirements include proficiency in the MS Office Suite, strong organizational skills and superb communication skills.Working Conditions:
