PURPOSE STATEMENT

Plan, direct, define and coordinate individualized and group therapeutic support and services to patient's with mental health, behavioral health and substance abuse issues in an outatient setting.

Responsibilities:

Provides a broad range of therapeutic and psychiatric social work services to mentally or emotionally disturbed patients. Acts as a liaison with hospitals, clinics, referral sources, and other medical professionals/centers affiliated with the facility. Collaborates with psychiatric and interdisciplinary teams in diagnoses and treatment plans. Assess relative, cognitive, behavioral or emotional problems. Other Duties as assigned

OTHER FUNCTIONS

Perform other functions and tasks as assigned.

Qualifications or Skill Required:

Master's degree in human services field. Must be licensed in the State of Illiniois; limited or full licensure welcomed.

Experience:

One-three years of experience or related experience, preferably in community mental health. Licenses, Certifications, registrations : Current State of Illinois LMSW (Licensed Master's Social Work) or LPC (Licensed Professional Counselor).
