The ideal candidate will provide essential support to nursing staff and assist patients in their daily activities, ensuring their comfort and well-being. This role is vital in delivering high-quality patient care within a patient home or facility, including specialized areas such as dementia care, hospice care. Duties Assist patients with personal hygiene tasks, including bathing, grooming, and dressing. Monitor and document patient vital signs and report any changes to nursing staff. Provide support in mobility assistance, including transferring patients to and from beds or wheelchairs. Aid in feeding patients who require assistance during mealtime. Maintain a clean and safe environment for patients by adhering to infection control protocols. Support nursing staff with medical procedures as needed, including catheter care and other basic clinical tasks. Engage with patients to provide emotional support and companionship, particularly in behavioral health settings. Utilize medical terminology effectively when communicating with healthcare professionals. Familiarity with electronic health record systems such as Cerner is preferred. Skills Experience in a hospice setting is highly desirable. Knowledge of dementia care practices to support patients with cognitive impairments. Understanding of hospice care principles to assist terminally ill patients compassionately. Familiarity with emergency medicine protocols for urgent patient needs. Strong communication skills for effective interaction with patients, families, and healthcare teams. Ability to work collaboratively within a multidisciplinary team environment.
