OVERVIEW OF POSITION
Under general supervision, works as part of an interdisciplinary team to improveclient outcomes by assisting families with health and social care resourceneeds. Oversees staffs outreach activities includingpartnerships, events, and support with obtaining resources. Contacts and coordinates with multiple functions including, but not limitedto: Autism Services Clinical, Clinical Training and Quality, Therapy Services, CustomerSuccess, and Living Options.
ESSENTIAL FUNCTION
Addresses families' immediate needs and provide assistance in obtaining services for better treatment outcomes. Assists families with administrative tasks to obtain services, including phone calls, collecting necessary documents, and follow-up. Receives, assesses, and engages with internal and external requests for services. Tracks staff engaging in community outreach activities and partnership development. Coordinates navigation, referrals, and follow-up for health and social care services with other providers, including community based organizations, to increase access to services. Maintains and completes record-keeping for services provided. Builds relationships with community organizations and agencies providing health and social care services. Develops and nurtures strong customer relationships for optimal health outcomes. Ensures outreach teams (BI Admins) adhere to timely customer success processes to internal and external customers. Sets clear expectations, evaluates staff performance, and provides feedback to ensure professional goals are being met. Attends community events and oversees outreach teams (BI Admins) for event coordination. Assists cross-departmental recruitment efforts by attending job fairs and other events to promote IDEA opportunities and initiatives Assists diagnostic clinics with recruitment, diagnostic processes, and follow-ups. Maintains a working knowledge of Community Health Worker practices and principles. Translates and/or coordinates material (e.g., flyers, brochures, forms) translation with translator. Other administrative tasks (e.g., meetings, documentation) and duties as assigned.
EDUCATION
Minimum of a high school diploma or
GED. EXPERIENCE
Typically requires 1-3 years of related experience within community engagement. Prior project lead and/or supervisory experience preferred.
KNOWLEDGE, SKILLS, ABILITIES
Working knowledge of Microsoft Office(Outlook, Word, Excel, etc.) and related computer software (for spreadsheets, word processing, virtual meetings, enter client data and notes, etc.). Ability to maintain positive customer service orientation and professionalism in all interactions. Ability to comuunicate effectively, through oral and written skills, and work cooperatively with a variety of individuals and groups. Must be able to work with diverse clientele and staff. Bilingual Spanish preferred. Ability to obtain and maintain a criminal record/fingerprint clearance from the Department of Justice and Federal Bureau of Investigation per Easterseals Southern California (ESSC) and/or program requirements. Must pass all drug testing required by ESSC.
