Job Description
Community Health Manager About us Greene County Health Department includes a passionate workforce of community-minded and health-oriented staff. About the role The Community Health Manager provides leadership, coordination, and operational oversight for community health programs, including Community Health Workers (CHWs), Peer Navigators, and other grant-funded initiatives. This position is responsible for ensuring high-quality service delivery, consistent documentation practices, staff supervision, work plan, and reporting alignment with grant requirements. The Manager plays a critical role in strengthening program infrastructure, supporting future billing readiness, and advancing sustainable service models that address the needs of rural communities in Greene County. All grant- and program-focused activities are based on the Greene County Health Department's Illinois Project for Local Assessment of Needs (IPLAN). Direct report to the administrator. Responsibilities Directly supervises Community Health Division staff and develops work plans based on community needs assessments, grant requirements, and program evaluation Tracks program deliverables, outcomes, and reporting deadlines to ensure compliance and performance Manages staff scheduling and daily operations, including coordination of outreach, navigation, and community-based services Coordinates program development using outreach data, partnerships, and community health priorities to improve outcomes for Greene County residents Develops programs, tools, resources, and solution-focused methods to connect clients to services that improve health and wellness Assists in developing infrastructure to support future billing for Community Health Worker (CHW) and peer services Identifies and implements evidence-based approaches to community health improvement and evaluates program effectiveness Develops and implements standard operating procedures (SOPs) to ensure consistency and quality across community health services Identifies service gaps and recommends program improvements to better meet community needs Ensures alignment of staff activities with community needs, grant objectives, and departmental priorities Strengthens coordination across program areas, staff, and service locations Assists with grant-related reporting, including required daily, monthly, and quarterly documentation Conducts other duties as assigned by the Administrator Candidate qualifications Bachelor's degree in Health Education, Social Work, Healthcare, Public Health, or a related field Experience supervising staff or coordinating programs in community health, public health, case management, or social services Experience working with grant-funded programs and reporting requirements Strong organizational skills with the ability to manage multiple priorities and programs simultaneously Effective written and verbal communication skills, including use of technology for documentation and communication Commitment to the mission of health prevention and community improvement Valid driver's license, reliable transportation, and current insurance Up-to-date immunization records Contact us to apply If you'd like to apply for this position, please complete and submit an application to the health department or email a copy to GreeneHD@greenecountyhd.org .
