About Consolidated Tribal Health Project, Inc. Consolidated Tribal Health Project, Inc. is a rural community health clinic that was incorporated as a 501 ( c )(3) in 1984 to address the disparity in health care among Native American people living in Mendocino County. Our Mission Consolidated Tribal Health Clinic, Inc. is committed to healthy lifestyles and honoring traditional Native American values to maintain physical, mental, emotional, spiritual and social wellbeing of the community. Job Summary- COVID 19 Vaccines/Booster Required. The Community Health Representative (CHR) serves as a liaison in our Native American community, working with patients, families and providers of Consolidated Tribal Health Project, to educate and provide information and support to improve overall health of the patient. The CHR conducts home visits to assist patients and families in understanding their conditions and treatment and are receiving appropriate care and support. The CHR participates in, planning and documenting Outreach Department activities.
DUTIES AND RESPONSIBILITIES
Coordinates with Consolidated Tribal Health Project departments to identify Native Americans in need of health care services. Meet with community members to help identify health care needs and adopt good health practices. Assist and support clients in obtaining services from CTHP and other community or health care resources. Monitor clients with known health conditions, provide education programs to community groups. Document all home visits and report significant findings to supervisor and provider. Coordinate client services with other departments at CTHP, make referrals, and participate in developing coordinated care plans. Organize and present planned health education programs to community groups. Participate in and document community participation in Outreach events, activities and projects. Advocate for community members in relation to their health care needs. Make reports as required by law, such as Report of Abuse. Attend continuing education seminars, workshops, conferences or trainings to maintain up to date information on health issues. Assist in planning Outreach Programs and organization activities. Develop and maintain resource and health education materials.
QUALIFICATIONS & REQUIREMENTS
High School diploma or GED. One - two years general work experience. Experience and interest working with elders and youth. Proof of valid California Drivers License. Excellent written, oral, communication and interpersonal skills. Ability to develop working relationships with community members and patients. Ability to maintain confidentiality of information. Current CPR or BLS (Must obtain within 3 months of hire). Computer proficiency in Microsoft Work, PowerPoint, Excel, Outlook, and other programs essential to the function of the department. Experience with HER and RPMS desirable. CHR Certification preferred. Indian Preference applies to all Consolidated Tribal Health Project positions in accordance with CTHP policy and applicable federal law.
