Job Description
Job Summary The Community Health Care Worker (CHW) serves as a trusted member of the community and a vital link between individuals, health/social service systems, and community-based resources. The CHW promotes health equity by building relationships, providing culturally competent health education, supporting care coordination, and advocating for the needs of underserved populations. The CHW will also engage with diverse populations, including individuals with disabilities, to empower them through knowledge, resources, and advocacy that improves overall well-being. Key Responsibilities Community Engagement & Outreach Establish and maintain trusting relationships with community members. Conduct outreach in homes, schools, community centers, faith-based organizations, and other local settings. Serve as a liaison between community members and health, social service, and governmental agencies. Conduct outreach activities to educate the community about available health resources and programs. Deliver presentations on health topics through public speaking engagements in the community. Health Education & Promotion Provide culturally and linguistically appropriate health education on topics such as chronic disease prevention, maternal/child health, nutrition, mental health, substance use, and healthy lifestyles. Facilitate workshops, group sessions, and one-on-one health coaching. Teach health-related workshops that promote awareness of medical terminology, nutrition, and healthy living. Utilize motivational interviewing techniques to encourage healthy behaviors and lifestyle changes. Care Coordination & Navigation Assist individuals in accessing medical care, behavioral health services, housing assistance, food support, and other community resources. Support clients in scheduling and attending appointments, understanding treatment plans, and adhering to medication regimens. Help individuals navigate healthcare systems, including Medicare, Medicaid, and public health services. Provide support for individuals with disabilities, ensuring they receive appropriate care and resources. Follow up to ensure continuity of care and provide support in overcoming barriers (transportation, language, financial, etc.). Advocacy & Capacity Building Advocate for the health and social service needs of the community within service agencies and institutions. Work collaboratively with healthcare providers, social workers, nutritionists, addiction counselors, and community partners to provide comprehensive support and address systemic barriers. Participate in community health assessments and contribute to local health initiatives. Data Collection & Reporting Document services provided and maintain accurate client records in compliance with organizational and funder requirements. Collect and report data for program evaluation and quality improvement. Qualifications High school diploma or equivalent required; associate or bachelor's degree in health, social work, or related field preferred. Prior experience working in community outreach, public health, or human services strongly desired. Lived experience and/or strong connection to the community being served. Knowledge of local health and social service systems, including Medicare and Medicaid. Bilingual or multilingual skills highly preferred. Skills & Competencies Strong communication and interpersonal skills, with cultural humility and respect for diverse backgrounds. Ability to build trust, motivate individuals, and maintain professional boundaries. Problem-solving and advocacy skills. Organizational and documentation abilities. Commitment to health equity and community empowerment. Public speaking and presentation skills. Basic computer skills. Working Conditions Community-based setting with significant fieldwork. Flexible schedule, including evenings and weekends as needed. Requires travel within service area (reliable transportation required).
