Community In Home Caregiver & Coordinator (PSS Certification Required)

Chebeague Cares, our personal care agency, is seeking a compassionate, organized Community Care Coordinator to help lead our in-home care program white also providing direct care. This unique hybrid role included at least 30 hours/week of hands-on caregiving, combined with 10 hours of coordination, involving staff support, scheduling and client oversight. Make a meaningful impact by helping community members remain safe, independent, and at home. What you'll do:

• Assist with client intakes, assessments & personalized care plans
• Coordinate schedules and support caregiving staff
• Build relationships with clients, families & community partners
• Assist with hiring, onboarding & training
• Provide consistent, high-quality service
• Provide a minimum of 30 hours/week of direct, hands-on care What you bring:
• 2+ years as PSS is a must. CNA, CRMA, or similar are welcome to apply
• Experience with direct care giving of elderly population, preferably in an in-home care environment
• Must be flexible, compassionate, highly motivated, professional and a positive person
• Strong leadership, communication & organizational skills
• Ability and willingness to balance direct care with coordination
• Valid driver's license & ability to pass criminal background checks Why you'll love this role:
