ADVANCED PRACTICE REGISTERED NURSE
(APRN-PCNS)COMPLEX CARE TEAM PART TIME
General Summary:
Provide integrated, patient centered care to individuals with complex medical and behavioral health needs. The APRN for the Complex Care Team plays and key role in stabilizing acute concerns, reducing care fragmentation and improving overall health outcomes.Essential Responsibilities:
- Prescribe and monitor appropriate medication and treatments to Thrive clientele in keeping with the Thrive's policies and procedures and under the supervision of Thrive's psychiatrist. Supervision in accordance with Thrive Advanced Practice Registered Nurse Supervision Protocol.
- Provide integrated medical and psychiatric assessment, diagnosis and treatments for patients with complex, high risk health needs.
- Monitor medical and psychiatric medications collaboratively, with careful consideration for interaction and comorbidities.
- Partner with RN care managers, case managers and primary care providers to create and adjust comprehensive, patient centered care plans.
- Support care transitions (hospital to home, ED follow up) to reduce readmissions and improve outcomes.
- Participate in case conferences and interdisciplinary team meetings.
- Serve as consultant on medical/psychiatric issues to other members of clinical treatment teams.
- Serve on Thrive committees such as Utilization Review and Quality Assurance, etc., as assigned;
- Incorporate the Thrive's mission and Guiding Principles in the performance of professional duties;
- Serve as an educational resource to staff, clients, and the general public regarding medical issues and their integration in a clients behavioral health needs.
Additional Responsibilities:
- Willing to participate in home visits
- Attend all agency/department required training and meetings within the prescribed period.
- Attend all Safety Training programs and be able to describe his/her responsibilities related to the general safety, department safety, and specific job-related hazards and responsibilities.

- Present a neat appearance in proper attire and identification as required by the position, department and agency policies and procedures.
- Work the hours and/or shifts assigned, and begin and end on time.

Qualifications Education, Experience and Competencies:
- Excellent clinical and communication skills. Non-hierarchical and flexible management style.
- At least five (5) years of related community mental health experience.
Certifications, Licenses Requirements:
- Masters prepared nurse with American Nurse Credentialing Thrive Certification, State of RI APRN License, State of RI Controlled Substance Registration Number, and a Federal DEA Registration Number.
- Preferred dual certification as a Psychiatric mental health NP and primary care specialty such as
FNP, AGPCNP
