Job Requirements Education Requirements:
Valid Licensed Practical Nurse (LPN) licenseCertifications Requirements:
BLS certification, ACLS & Wound Care certification (preferred but not mandatory)Experience Requirements:

Job Responsibilities Provide compassionate and efficient nursing care toLTAC/LTC
patients, adhering to facility protocols and patient care plans. Monitor patient health status, administer medications, and document responses accurately within electronic medical records (EMR). Collaborate with interdisciplinary teams to develop and update patient care plans that meet individual needs. Maintain compliance with all safety and healthcare regulations, including BLS and other required certifications.
