Job Summary:
The Brain Injury Waiver (BIW) Service Coordinator is responsible for coordinating, scheduling, and monitoring in-home care services for individuals enrolled in Missouri's Brain Injury Waiver program. This role serves as the primary point of contact for participants, families, and care providers to ensure services are delivered effectively, safely, and in compliance with state regulations and agency standards. The Service Coordinator supports participant independence and satisfaction by managing care plans, addressing service needs, and maintaining strong communication across all stakeholders.
Essential Duties and Responsibilities:
Implement state-authorized service plan for attendant care services. Ensure services are delivered according to approved hours, task and waiver guidelines. Educate participants and attendants on service expectations and responsibilities. Maintain accurate daily and weekly schedules for assigned attendants to ensure uninterrupted client services. Coordinate and assign/schedule attendants based on the participant's authorized care plan. Ensure appropriate staffing coverage, including backup plans for call-offs, emergencies, missed visits and staffing shortages Maintain ongoing communication with attendants to address scheduling changes. Verify attendants meet qualifications and training requirements as applicable. Monitor authorized services hours and notify supervisors of potential over-utilization or services gaps. Track and document missed visits and report scheduling issues to the Director of
ADS/BIW QUALIFICATIONS
Minimum one (1) year experience in service coordination, home care scheduling, case management, or long-term services and supports Knowledge of Medicaid or Home and Community-Based Services (HCBS) preferred. Strong organizational and scheduling skills. Effective communication and problem-solving abilities. Customer Service focused. Valid driver's license and reliable transportation for required home visits. Physical Requirements Lift and carry up to 25 pounds Ability to sit, stand, walk, bend, reach and perform repetitive hand and keyboard movements for extended periods. Work Environment Work is combination of office-based scheduling and field-based monitoring visits. Regular travel withing assigned service area. May require flexibility to address urgent staffing or service's needs. Pay:
$19.00 - $20.00 per hour
