Job Description

Client Services and Marketing Manager Forest, MS 39074 $17 an hour - Part-time $17 an hour - Part-time Client & Community Relations Specialist The Client & Community Relations Specialist is responsible for growing the agency's client census through client intakes, community outreach, networking, and referral development. This position serves as a liaison between Heart of Grace Home Care and referral sources, clients, families, healthcare professionals, and community organizations. Responsibilities include receiving and processing new client referrals, completing intake calls, coordinating the start of services for new clients, and following up with prospective clients and families. The Client & Community Relations Specialist will build and maintain relationships with case managers, hospitals, nursing facilities, rehabilitation centers, physicians, social workers, veterans organizations, and other referral sources. This position is responsible for representing the agency in the community by attending and coordinating networking events, health fairs, job fairs, community resource fairs, open houses, and other outreach activities. The specialist will promote agency services, increase community awareness, develop referral partnerships, track referral sources, and identify opportunities for agency growth. The ideal candidate will possess strong communication, organizational, and relationship-building skills; have reliable transportation; be willing to travel throughout the service area; and represent Heart of Grace Home Care in a professional manner at all times. Performs other duties as assigned by management.
