Our clients rely on us for personal care, medication reminders, and meaningful companionship—and we rely on dedicated professionals who believe deeply in the impact of caregiving. We are seeking a Community & Client Care Manager to serve as a key connector between clients, families and caregivers. This role blends client intake and assessment and community-based care . The ideal candidate understands the senior care landscape, values relationship-driven growth, and is passionate about supporting both clients and caregivers. Key Responsibilities Conduct in-home assessments and client intakes throughout the Portland metro area; primarily eastside focused. Guide families through the care-planning and onboarding process with empathy and professionalism Develop and maintain strong relationships within the community of Courtyard at Mt.

Tabor. Collaborate closely with management and caregiving teams to ensure exceptional care delivery Serve as an advocate for clients and caregivers, recognizing the vital role caregivers play in our mission Ideal Candidate Profile Experience in the senior care or home care industry (caregiving experience strongly preferred) Proven ability to build trust and long-term professional relationships Personable, confident, and approachable with excellent communication skills Active listener who is coachable and solutions-oriented Driven, persistent, and comfortable with community outreach and relationship-based growth Professional, flexible, and mission-driven Position Details This is a full-time, salaried position with the opportunity to work alongside one of the most supportive and purpose-driven teams in the industry.
