Bridges Home Services provides in-home services designed to help vulnerable adults live independently and securely in their own homes by providing personal and domestic care services.
Responsibilities of this position include:
Referral and Marketing:
taking initial calls about inquiries to services, describing services and following up with potential referrers, scheduling initial visits to admit clients to care
Scheduling:
is responsible for ensuring that client's allotted or desired scheduled hours are staffed with the most qualified employee, that overtime is not accumulated, and that staff's schedules are communicated and documented appropriately. Also responsible for ensuring that client schedules are met as authorized by funding sources to include Section 21 and Section 29.
Contracts:
ensuring that services are being delivered per funding source authorizations and consumer contracts including completing 6 month and annual home/ zoom visits, ensuring service authorizations are up to date, that schedules are communicated appropriately, and that hours provided are the same as hours contracted.
Payroll:
ensuring staff have worked as scheduled by ensuring scheduled visits have been claimed in the computer system, trouble-shooting when something goes amiss.
Care Planning and Personal Goals:
assisting client with developing plan of care, adjusting care plan as needed, and ensuring that care plan is being met through telephone calls and in-person home visits as mandated per funding source. Entering information in EIS and managing 6month and yearly meetings with consumers and case managers to ensure these goals are being met.
Supervision:
interviews, hires, supervise, counsels, terminates and schedules Direct Support Professionals.
Problem Resolution:
As a primary contact point for all callers to Bridges Home Services In-Home Supports, customer service and customer relations are of the highest priority in this position.
Documentation:
ensuring schedules are updated and current in the scheduling system, ensuring conversations with staff, consumers, and family are documented in client management system; all charts are up to date and have updated forms and home visit notes.
Reporting:
reports concerns to the family, Adult Protective Services, the Department of Health and Human Services, supervisor, and provides necessary information to internal staff. Requirements A.A. in Business Administration or related subject or High School Degree and equivalent experience. 2 years progressively more responsible administrative work experience. Experience working in health care preferably related to aging issues, scheduling and assigning home services and customer satisfaction. Experience with one or more automated scheduling data bases highly desired.
