The Client Care Manager plays a key role in creating meaningful connections with our clients and ensuring they receive thoughtful, personalized support every step of the way. This position blends relationship-building, problem-solving, and coordination to deliver an exceptional care experience for both clients and caregivers.
What You'll Do Build Relationships:
Meet with new clients and families, listen to their needs, and create personalized Care Plans that truly reflect their goals and preferences.
Support Ongoing Care:
Regularly check in, visit clients, and adjust Care Plans as needs change to ensure the highest level of care. Coordinate with the
Team:
Work closely with our staffing team to match clients with the right caregivers and set everyone up for success.
Ensure Quality & Satisfaction:
Be a trusted point of contact for clients and families, addressing concerns and celebrating wins along the way.
Support Our Caregivers:
Participate in caregiver feedback and evaluations to help our team grow and maintain our care standards.
Stay Organized:
Keep accurate, up-to-date records to ensure smooth communication and continuity of care. Be Part of the
Team:
Attend meetings, trainings, and contribute to a positive, collaborative workplace culture. What We're Looking For Experience in care management or a related field A natural ability to connect with people and build trust Strong organization and communication skills Comfort with technology and scheduling systems A self-starter who can manage a fast-paced environment A compassionate, flexible, and solution-oriented mindset Schedule & Flexibility This role is primarily Monday-Friday, 8:00 AM - 4:30 PM. Because care doesn't always happen on a schedule, there will be times when you'll need to step in outside of business hours to support clients and caregivers when it matters most.
