Job Description
Job Summary The Client Care Coordinator is responsible for ensuring exceptional service delivery by managing the scheduling and care needs of clients in a home health care agency. This role involves coordinating client services, acting as a liaison between clients, caregivers, and the office, and maintaining compliance with agency standards. The ideal candidate is organized, detail-oriented, and committed to enhancing client satisfaction while ensuring smooth daily operations. Key Responsibilities Client Coordination Serve as the primary point of contact for clients and their families, addressing inquiries, concerns, and service updates. Conduct initial client assessments in collaboration with clinical staff to develop personalized care plans. Match clients with caregivers based on skill set, availability, and client preferences. Monitor client satisfaction and service quality, addressing concerns promptly. Scheduling and Staffing Create and maintain caregiver schedules, ensuring adequate coverage for all shifts. Coordinate replacements for last-minute cancellations or absences. Communicate schedule changes to clients and caregivers in a timely manner. Caregiver Support Serve as a resource for caregivers, answering questions and providing guidance. Facilitate onboarding for new caregivers, ensuring proper orientation and training. Assist in resolving conflicts or issues between caregivers and clients. Administrative Duties Maintain accurate records of client care plans, caregiver assignments, and schedules. Ensure compliance with HIPAA regulations and agency policies regarding client information. Update and track client files in agency software systems (e.g., Sandata, DeVero, or Care Connect). Participate in regular team meetings to discuss client updates and staffing needs. Compliance and Quality Assurance Ensure compliance with state and federal regulations, including EVV requirements. Collaborate with clinical staff to address changes in client care needs. Monitor caregiver certifications and training compliance in coordination with HR. Qualifications Education and Experience High school diploma or equivalent (required); Associate's or Bachelor's degree in healthcare administration, social work, or related field (preferred). Experience in scheduling, client coordination, or a similar role in healthcare (1-2 years preferred). Skills and Competencies Strong organizational and time management skills. Excellent verbal and written communication skills. Ability to work under pressure and handle multiple priorities. Proficiency with scheduling software and EHR platforms (e.g., DeVero, Sandata, or Care Connect). Problem-solving skills with a customer service mindset. Knowledge of HIPAA regulations and home health care industry standards. Additional Requirements Ability to work flexible hours, including occasional evenings or weekends, as needed. Valid driver's license and reliable transportation (if home visits are required). Background check and drug screening clearance. Physical Requirements Prolonged periods of sitting at a desk and working on a computer. Occasional travel to client homes or other agency locations (if applicable). Work Environment This position operates in a professional office environment with occasional exposure to client homes and caregivers in the field.
