Job Description

Job Overview We are seeking a dynamic and compassionate Client Care Coordinator to join our healthcare team! In this vital role, you will serve as the primary point of contact for patients and clients, ensuring their experience is smooth, welcoming, and efficient. Your energetic approach will help foster positive relationships, coordinate appointments, and support the overall administrative flow of our medical practice. Duties Greet patients and clients warmly, creating a positive first impression Schedule appointments and manage calendar coordination for multiple providers Verify patient information and update records accurately using administrative support tools Communicate clearly with patients regarding appointment details, follow-up instructions, and necessary documentation Maintain organized patient files and ensure confidentiality in accordance with privacy regulations Assist with verifications and Plays an integral role in interfacing with clients and families to ensure safe, appropriate and meaningful care is being provided Serves as a mentor to caregivers and supports their care delivery. Develops and maintains the client care plan, creating extraordinary client experiences and communicating with clients, families, care partners and referral sources. Maintains a calm, positive demeanor at all times and is able to effectively multitask in a high functioning office environment. Experience Strong knowledge of computers is a must in order to facilitate effective communication and documentation High school graduate or equivalent with two years of business experience. The ideal candidate has prior experience working in the home care industry or medical office setting Proven ability to provide excellent customer service able to work independently, demonstrating sound judgment. Pay:
$15.00 - $27.74 per hour
