This is an office-based position focused on client advocacy, service coordination, and helping individuals access the support and resources they need to live successfully in the community. The Client Services Coordinator serves as a primary point of contact for clients and their families, helping address concerns, coordinate services, connect clients with community resources, and ensure client needs are communicated effectively to internal staff and external partners. This position does not provide direct caregiving or hands-on care. Instead, the role focuses on client support, problem-solving, communication, and service coordination from our office. Key ResponsibilitiesClient Advocacy & Support Serve as a primary point of contact for clients and families. Build positive relationships with clients through regular communication and follow-up. Advocate for client needs and help resolve service-related concerns. Assist clients in accessing community resources such as housing assistance, food support, transportation resources, healthcare services, and other available programs. Monitor client satisfaction and identify opportunities to improve services. Service Coordination Coordinate with case managers, social workers, guardians, family members, and community partners. Ensure client services align with authorized care plans and individual needs. Assist with onboarding new clients and conducting welcome calls. Communicate service updates and changes to clients and relevant stakeholders. Escalate urgent client concerns to management when necessary. Communication & Relationship Management Answer incoming calls, emails, and text messages professionally and promptly. Maintain regular contact with clients to ensure ongoing needs are being met. Provide updates regarding services, resources, and agency programs. Represent the agency professionally when communicating with referral sources and community partners. Documentation & Administrative Support Maintain accurate client records and communication logs. Document client concerns, requests, and resolutions. Assist with service verification, forms, and client-related administrative tasks. Ensure documentation is completed accurately and in a timely manner. Team Collaboration Work closely with office staff, designated managers, staffing coordinators, and case managers. Participate in team meetings and contribute to service improvement initiatives. Support agency operations and special projects as assigned. Qualifications Excellent communication and interpersonal skills. Strong customer service and relationship-building abilities. Ability to manage multiple priorities and follow through on tasks. Strong problem-solving and critical-thinking skills. Highly organized with strong attention to detail. Comfortable working with diverse populations and individuals with varying support needs. Proficiency with Microsoft Office, Google Workspace, CRM systems, and general office technology. Experience in healthcare, homecare, social services, case management, housing services, human services, or customer service preferred. High school diploma required; additional education in human services, healthcare, or related fields is a plus. Preferred Characteristics Compassionate and client-focused. Strong advocate for individuals receiving services. Professional and dependable. Resourceful and able to navigate community support systems. Comfortable communicating with clients, families, case managers, and community partners. Important Note This is an office-based coordination and client advocacy position. Employees in this role do not provide direct caregiving, personal care, homemaking, or hands-on support services to clients.
Pay:
$19.00 - $26.00 per hour
