Full Job Description Job Purpose:
This position ensures high-quality service delivery and compliance for LifeCare Alliance's Nutrition Programs by combining client eligibility assessment with quality assurance oversight. As a Meals on Wheels Client Services Coordinator, you'll be the friendly face that guides clients through the meal intake process, ensuring they receive the nutrition they need while keeping everything running smoothly behind the scenes.
Key Responsibilities:
Conduct meal client intake and assessment to determine eligibility and service needs. Collaborate with the assessment team to address observation reports and process internal referrals to other LifeCare Alliance departments for clients. Monitor quality assurance and service delivery to ensure high standards are met. Develop process improvement recommendations to enhance the program's efficiency and effectiveness. Work closely with clients, especially older adults and disabled/chronically ill individuals, to ensure their needs are met with empathy and professionalism.
Required Education:
High school diploma or equivalent
Preferred Education:
Associates degree in social work, human services, or Community Health Worker certification
Required Experience:
Minimum of 2 years' experience working for a not-for-profit, community services
Preferred Experience:
Experience working with a government agency Experience working with older adults and disabled/chronically ill individuals
Required Skills and Abilities:
Proven documentation and process management skills
Preferred Skills and Abilities:
Detail-oriented and able to work in a fast-paced, deadline-driven environment
