The Client Care Associate provides direct support and assistance to individuals with disabilities or creative aging support. This role helps clients achieve greater independence by assisting with daily living skills, connecting to community resources, and supporting individualized service plans. You will engage in meaningful activities that promote development and enhance quality of life.Key Responsibilities:
Adheres to the vision, mission, and core values of KANDU Industries, Inc. in all aspects of the job. Keep an accurate record of client services including all necessary documentation related to legal records, medical records, consents, and authorizations Provide direct care and support to clients in community, or clinical settings. Assist with daily living tasks such as hygiene, medication reminders, transportation, and meal preparation. Support implementation of individualized service or treatment plans. Monitor client progress and document behavior, needs, and incidents. Assess consumer strengths and interests and assist with systematic instruction Evaluate and document client progress, including issues of performance, behaviors, and/or medical concerns Assist in crisis intervention and de-escalation when necessary. Complete written progress reports and service plans to be submitted to care teams, counselors, and funding sources promptly. Collaborate with case managers, clinicians, and other service providers. Participate in team meetings and training sessions to maintain program compliance and improve service delivery.Qualifications:
High school diploma or GED required; Associate's degree in Human Services or related field is a plus. Equivalent experience will be considered. Experience working with individuals with disabilities, mental health needs, or in social services. Strong interpersonal, communication, and organizational skills. Ability to work independently and as part of a team. Valid driver's licensePay:
