Job Description
Overview:
Directs daily operations of assigned supportive living (group) homes, currently located in Bellevue, Flat Rock, and Huron Ohio, overseeing FRCS consumers with intellectual/developmental disabilities as well as staff serving them. Under the direction of the Administrator (DOO), ensures assigned homes comply with Ohio DODD and Medicaid Certification standards and regulations as an agency provider of waiver services. Inspires and leads staff with positivity, involving them in the organization's mission, vision, values, and strategic goals.Supervision and Oversight:
Responsible for overseeing staff working in assigned supportive living homes. Directly supervises Direct Support Professionals (DSPs) and Lead DSPs in each assigned home. Guides and trains staff regarding compliance matters.Location:
Based in various counties served by the supportive living program however this particular position is responsible for three supportive living counties which currently totals 4 homes that are located in Bellevue, Flat Rock and Huron. While there's some schedule flexibility, this is a critical position requiring on-site work at supportive living homes. Local workday travel between homes is required with occasional regional, workday travel for meetings, events, conferences and/or to assist with consumer needs throughout, primarily, northern Ohio. Mileage reimbursement is provided for all company-related travel outside of the daily commute from home to primary work location. Overnight travel is extremely rare and voluntary.Schedule:
Hourly with primarily Monday - Friday/office hours with frequent interaction with homes staff who work mainly evenings/nights, 7 days/week. Emergency calls/messages may be after usual office hours. While many emergencies can be addressed by phone/video or other staff during off hours, occasionally, the Supportive Living Manager may need to report in-person to assigned homes to assist in these situations. Participates in a regular rotation of being the designated weekend on-call contact for homes and must be accessible during these times. If needed to cover any of the sleep shifts, there is a different rate for those hours (midnight to 6 am) however this would likely be rare if ever. The sleep shift rate is $15.00. Oversees operations of assigned homes, ensuring quality care and services for consumers. Maintains compliance with rules/guidelines for DODD/Medicaid Certification Standards and state/federal laws and regulations as well as Flat Rock's policies and procedures. Cooperates with County Board of Developmental Disabilities' Investigative Agent on the investigation of Major Unusual Incidents (MUIs), including any allegations of abuse/neglect, related to assigned homes. Ensures consumers' needs are met as outlined in their Individual Service Plans (ISPs). Monitors and remedies Quality Assurance issues in assigned homes. Performs regular visits to each home to review cleanliness, quality of care, and accuracy of documentation, files, physician's orders, financial records, etc. Attends ISP and other planning meetings for assigned consumers and provides input as a member of their team. Communicates with each consumer's team, including guardians (if applicable) and/or County Board of Developmental Disabilities. Advises Administrator (DOO) of issues that could adversely impact quality of care or compliance. Coordinates with Administrator (DOO) and the supportive living team, including other Supportive Living Managers, Lead DSPs, and DSPs regarding care and services provided in assigned homes. Maintains confidentiality.External Stakeholders:
Creates/sustains positive relationships with external stakeholders, including staff of County Boards and DODD, consumers, guardians, and other stakeholders. Ensures FRCS is viewed as a provider of creative solutions. Reviews ISPs for possible admissions to fill home vacancies, responds to inquiries about vacancies, provides tours/visits, and coordinates other logistics with consumers and County Service and Support Administrators (SSAs). Reviews potential admissions with the Administrator (DOO) for final approval.Leadership of Staff:
With Human Resources and the Administrator (DOO), creates a positive, engaging interview, onboarding, and new hire experience for staff. Works to increase onboarding success and new hire retention. Creates schedules to meet consumers' needs while balancing the self-care needs of staff. Ensures adequate, safe staffing for programs as well as assigned homes. May reassign staff based on emergent needs to meet the demands of all FRCS supportive living homes. Clearly and effectively communicates with staff and others. Effectively trains, coaches, engages, and motivates staff. Conveys leadership's gratitude for staff's work. Fosters a high level of staff engagement with mission, vision, values, and strategic direction. Leads by example. Coaches and mentors staff, creating a high-performance, engaged culture with reduced turnover and improving retention of rock star employees. Coaches, mentors, and supports Lead DSPs in effective, motivational, and engaging leadership of staff. Directly supervises Lead DSPs and DSPs and evaluates performance. Addresses opportunities for each staff's improvement and coaches for improved performance. Ensures all staff are compliant with training requirements. Encourages high performance by staff. Helps them understand consumers' needs and how they can make a difference.Financial Management:
o Working with the Administrator (DOO), manages financial performance of assigned homes. Learns and understands Medicaid funding streams. Manages assigned homes so that all billable services are appropriately captured. Eliminates waste and reduces costs appropriately without sacrificing quality. Works with the Administrator (DOO), to improve FRCS program financial performance. o Provides appropriate and accurate management of consumers' financial matters, ensuring bills are paid timely and accurately.Other:
o Completes mandatory training as scheduled, including certification in first aid, CPR, medication administration, trauma-informed care, etc. o Provides back-up to other Supportive Living Managers as needed. o May fill in homes when staff are unavailable or reassigned to higher priority needs. o Performs essential duties/responsibilities of the position with/without reasonable accommodation. o Other duties as assigned.Requirements:
Must be able to meet the following qualifications and any others required in the Ohio Revised Code for Provider Certification of Agency Providers, including: o A valid Ohio driver's license with an acceptable driving record, auto insurance, and motor vehicle. Mileage reimbursement is provided for all company-related travel outside of the daily commute from home to primary work location. o A high school diploma or certificate of high school equivalent. o At least two years of verifiable/documented full-time (or equivalent part-time amount) paid work experience as a direct support professional (or similar). o At least one year, preferably, of verifiable/documented full-time (or equivalent part-time amount) paid work experience in supervision of employees and/or financial management of a department or work unit.Must be able to:
o Work positively and effectively with individuals with intellectual/developmental disabilities, including those who may have high behavioral needs that may cause them to act aggressively or combatively. o Read, write, and understand English at a level sufficient to comply with all requirements set forth in administrative rules governing the services provided by an agency provider. o Successfully pass a background investigation in accordance with rule 5123-2-02 of the Ohio Administrative Code and consent to be enrolled by the Ohio Department of Developmental Disabilities in the Ohio Attorney General's retained applicant fingerprint database known as Rapback. o If hired, shall complete all training as required by the Ohio Department of Developmental Disabilities' (DODD). o Must not have previously served as Administrator/CEO/Director of Operations for any agency provider within the last five years that had its provider certification revoked/not renewed.Desired Candidate Attributes:
