Direct Service Worker (DSW)

A Direct Service Worker (DSW) provides services to individuals in their homes and out in the community, whose needs vary from assistance with caring for themselves in regular daily activities, non-skilled health monitoring, to behavioral redirection/management with those consumers who may suffer from mental disabilities. Direct Service Workers are responsible for ensuring that services are delivered in a caring and respectful manner as well as in accordance with relevant agency policies and industry standards. Direct Service Workers are to report to the Field Supervisor(s). Shift is 12 hours a week. The schedule is flexible. Qualifications/Education 18 years or older High School Diploma, GED, or paid work experience Current First Aid (must be obtained within 30 days of being hired) Driver's License/Identification Card Vehicle Registration Valid Car Insurance Responsibilities/Activities Bathing

• Assist to medical appointments Ensure safety and security by supervising the home environment Provide the direct care services to the client at the time and place assigned Carry out duties as assigned by the field supervisor Call in/ out to the field supervisor if you are assigned According to care plan if you have to escort the client in an outing, you are to call the field supervisor anytime you leave and are still on the clock Report any abuse, neglect, or exploitation of clients to a supervisor Must attend orientations, in-service training, and staff meetings Be responsible for daily documentation of services provided and status of the client to be reported on progress notes & ALL
