Piatt County Mental Health is a small rural agency with big city impact on the individual's served. Our primary focus is on assisting individual's with developmental disabilities in achieving their hopes and dreams. Responsibilities Provide direct care and support to individuals with developmental disabilities in home and community settings. Assist clients with daily living activities, including personal hygiene, meal preparation, and medication management. Observe and report changes in clients' physical and emotional conditions to appropriate personnel. Facilitate social interactions and community involvement by accompanying clients on outings and activities. Maintain a safe and supportive environment that encourages independence and personal growth. Document client progress and maintain accurate records of services provided. Requirements

• High School diploma
• functional literacy
• driver's license
• Pass a Uniform Conviction Information Act criminal history record check pursuant to the Health Care Worker's Background Check Act, drug screen, physical and Illinois Sex Offender Registry check.

We will provide training to the right candidate.
