The ideal candidate will play a crucial role in enhancing the quality of life for our clients by assisting them in daily activities, promoting independence, and ensuring their overall well-being. This position requires a commitment to fostering a supportive and inclusive environment. Duties Assist individuals with daily living activities, including personal hygiene, meal preparation, and medication management. Provide support to individuals in group homes or community settings. Implement individualized care plans tailored to the specific needs of each individual. Monitor each individual behavior and report any changes or concerns to the appropriate staff members. Ensure compliance with HIPAA regulations and maintain confidentiality of individual information. Participate in training sessions in our Relias Training, CPR, First Aid, and other relevant certifications. Facilitate social interactions and community engagement for individuals, promoting inclusion and participation in various activities. Assist with transportation needs for individuals as required. Experience Previous experience in home health care, group home settings, or working with individuals with developmental disabilities is preferred. Knowledge of patient monitoring techniques and home community care is beneficial. Certification in CPR and First Aid is highly desirable. Strong communication skills and the ability to work effectively as part of a team are essential.
