This is a key leadership role within our skilled nursing facility focused on supporting residents and families through every stage of their healthcare journey, from admission through discharge planning, while promoting dignity, emotional well-being, resident rights, and quality of life. We are looking for a strong social services professional with Skilled Nursing Facility (SNF) / Long-Term Care experience
REQUIRED
who understands the importance of resident advocacy, interdisciplinary collaboration, care planning, and regulatory compliance. REQUIRED
Current Texas Social Work license (LBSW or higher)
REQUIRED
— must be active and in good standing Minimum 2 years Skilled Nursing Facility (SNF) or Long-Term Care experience
REQUIRED
