Discharge Planner

​ Southwood Psychiatric Hospital offers comprehensive mental health treatment programs for children and adolescents ages 3-21 whose lives have been disrupted by mental illness. Our facility located in the South Hills of Pittsburgh, Pennsylvania, provides many levels of care. Currently we are looking to hire a Discharge Planner.

PURPOSE STATEMENT

​ Provide assistance to patients and their families by developing a docu mented continuing care plan to en sure ongoing support in r ecovery and or other behavioral , mental or health care follow up after discharge from the facility . ​

ESSENTIAL FUNCTIONS

​ D evelop continuing after care plan , b ased on patient's needs, to promote long term health and relapse prevention . ​ Responsible for setting discharge appointments for patients , including group or individual therapy, psychologist or psychiatrist. ​ Make arrangements for community resources to facilitate discharge. Provide all needed documentation to other providers to facilitate the continuum of care. ​ Act as a lia i son between referral sources, patients and counselors. ​ Ensure all patient discharge planning documentation is clear, concise and timely . ​

EDUCATION/EXPERIENCE/SKILL REQUIREMENTS

​ Associates degree in nursing or human services field required . Bachelor's d egree in social work or human services field preferred . ​ One or more years' experience in a clinical setting preferred. ​ O ne or more years ' experience in discharge planning preferred. ​

LICENSES/DESIGNATIONS/CERTIFICATIONS

​ If applicable, current licensure as a LPN, RN within the state where the facility provides services . ​ CPR and de-escalation and restraint certification required (training available upon hire and offered by facility . ​ First aid may be required based on state or facility requirements. ​ We are committed to providing equal employment opportunities to all applicants for employment regardless of an individual's characteristics protected by applicable state, federal and local laws.
