Job Description:
Other duties as assigned. Develops and delivers educational materials and training programs for Care Management. Collaborates with Care Management leadership to identify educational gaps and create campus-specific educational plans. Conducts New Hire Care Management training to ensure new care managers receive necessary education and tools. Mentors and trains care management team members to enhance workflow processes and service quality. Supports performance improvement activities through problem identification, assessment, data gathering, change implementation, and evaluation of outcomes. Participates in Process Improvement Initiatives and ensures practice changes are implemented. Audits care management compliance with regulatory regulations and documentation standards. Reviews and revises Care Management Policies, SOPs, and Job Descriptions as needed. Identifies community resources and coordinates education opportunities for Care Management staff.
Knowledge, Skills, and Abilities:
  • Ability to lead in the delivery of Care Management education [Required]
  • Ability to lead performance improvement and strategic initiatives [Required]
  • Ability to think critically in relation to the operations and delivery of patient care [Required]
  • Ability to identify gaps in Care Management process and develop appropriate education plans [Required]
  • Excellent social or clinical practice knowledge and can apply the knowledge to development and delivery of Care Management education [Required]
  • Acts as a role model for professionalism for the Care Management system [Required]
  • Excellent ability in making presentations to management, staff, and business partners [Required]
  • Comfortable in a complex and changing healthcare environment [Required]
  • Medical record investigative review knowledge [Required]
  • Effective oral and written communication skills, with the ability to articulate complex information in understandable terms to all levels of staff [Required]
  • Effective computer skills, particularly Microsoft Office Outlook, Word, Excel [Required]
  • Familiarity with Center for Medicare and Medicaid Services Programs [Preferred]
  • Knowledge of Cerner NaviHealth; Suncare; Qlikview [Preferred]
  • Ability to read, interpret, and strategize future projects [Preferred]
Education:
  • Master's [Required]
  • Bachelor's of Nursing [Required]
  • Master's [Preferred]
Field of Study:
  • in Social Work
  • in Nursing Education or health care related field
Work Experience:
  • 3+ functional healthcare setting [Required]
  • In Case Management, Utilization Management, Transitional Care [Required]
  • In education in a healthcare or Care Management setting [Preferred]
  • With Microsoft Office, including Excel, PowerPoint, and Word [Required]
