Job Description About JARC:
JARC is a Michigan based 501(c)(3) non-profit organization that provides high-quality, community-based residential and support programs, serving nearly 200 adults in over 20 group homes and various supported independent living arrangements.
About this position:
The Home Health Education Coordinator provides operational, administrative, and staff support services to ensure efficient day-to-day operations for our persons served. This role assists with routine care procedures such as administering medications, checking blood sugar levels, changing bandages, etc. The ideal candidate is highly organized, detail-oriented, comfortable using technology and able to work effectively in both office and field settings. The Home Health Education Coordinator plays an important role in supporting staff performance, training readiness, and quality service delivery.
Responsibilities:
Field & Employee Support Assess and chart observations of persons served condition. Track outcomes and make recommendations for process improvement. Review medication and vital signs. Collaborate with medical professionals to ensure proper health and wellness of persons served. Provide education to staff on proper health care procedures and strategies and serve as a liaison. Coordinate persons served care plan. Provide support to employees and leadership in both office and field environments. Conduct home visits as needed to assist with operational coordination and staff support. Help address persons served concerns and escalate issues appropriately. Maintain employee records, certifications, training documentation, and compliance files. Schedule training, meetings, staff support visits, and follow-up activities. Assist with coordinating CPR and staff development training. Technology & Documentation Utilize electronic systems, scheduling platforms, databases, and MS Office applications. Maintain accurate data entry and documentation practice.
Work Environment:
Combination of office and field-based work. Local travel is required to support operational and training activities. May require occasional schedule flexibility to support workforce needs. High school diploma or equivalent required. Associate degree, Certified Nurse Assistant (CNA), Licensed Practitioner of Nursing (LPN), or equivalent required. Previous experience in healthcare support, nursing education, administration, or workforce support preferred. Minimum 5 years' experience in residential home health care. Previous experience conducting First Aid/AED/CPR Training. Strong organizational, communication, and problem-solving skills. Proficiency in MS Office and electronic documentation systems. Ability to maintain confidentiality and professionalism. Valid driver's license and reliable transportation required.
