  • 4:00 PM LSW Required Conduct assessments, coordinate admissions, and help clients access treatment services in a fast-paced behavioral health setting.
This role serves as the primary coordinator of the assessment and admissions process for our Outpatient, Intensive Outpatient (IOP), and Partial Hospitalization (PHP) programs. We are seeking a highly organized, responsive, and clinically skilled professional who can independently manage referrals, complete assessments, coordinate admissions, and maintain timely documentation.
What You'll Do Assessment Services:
Conduct comprehensive biopsychosocial assessments for substance use and co-occurring mental health disorders. Complete Mental Status Examinations (MSEs), screening tools, and clinical assessments. Utilize DSM-5 criteria to formulate diagnostic impressions and recommendations. Utilize ASAM Criteria to determine appropriate levels of care and treatment recommendations. Complete assessment documentation within 24-48 hours of service delivery.
Admissions Coordination:
Serve as the primary point of contact for new referrals. Schedule assessments and coordinate admissions into treatment services. Verify insurance eligibility through the Electronic Health Record. Coordinate schedules with counselors, case managers, psychiatry, nursing, and other treatment staff. Obtain releases of information and referral documentation. Ensure clients are successfully connected to treatment following assessment.
Referral Source Communication:
Communicate with courts, probation officers, attorneys, child welfare agencies, hospitals, employers, and community providers. Provide updates and documentation as permitted by releases of information.
Documentation & Compliance:
Maintain accurate and timely documentation within the Electronic Health Record. Ensure compliance with Ohio Medicaid, Joint Commission, HIPAA, and 42 CFR Part 2 requirements.
