• Lebanon, NH Dartmouth Health's Department of Community and Family Medicine is recruiting for a Family Medicine APRN or PA to join our academic Primary Care practice at our Heater Road Clinic in Lebanon, NH.

The Heater Rd. team is located in a beautiful outpatient facility three miles from Dartmouth-Hitchcock Medical Center. We are currently seeking candidates that have 3+ years of primary care provider experience, for this role. The Family Medicine team at Heater Road is an integral part of a dynamic Primary Care group that has a focus on clinical and educational quality improvement, population health, and the development of new models of care delivery. We strive to provide team-based, high-value quality care to all patients within an academic setting where teaching, learning and scholarship is central to our work and mission. Unique aspects of our practice include Family Medicine physicians who have additional training in Sports Medicine, Sleep Medicine, Functional Medicine, and Addiction Medicine, and we provide the potential for leadership development opportunities. Our integration within Dartmouth-Hitchcock Medical Center provides for unique collaborative relationships across an extensive array of specialties, as well as tremendous technical and administrative support. • Graduate of an accredited APRN or PA academic program
• NCCPA, AANP or ANCC certified Required Licensure/Certifications
• Eligible for RN licensure in the State of NH (APRN only)
• Eligible for APRN or PA licensure in the State of NH
• Eligible for Federal DEA Registration
