Reimagine Aging Care. Lead with Purpose. Geriatric Medicine Physician Faculty Scranton, PA | The Wright Center for Community Health & Graduate Medical Education At The Wright Center, we believe health care is a human right
  • and how we care for older adults defines the strength of our communities. We're seeking a board-certified Geriatrician to step into a high-impact faculty and leadership role, advancing population health, age-friendly care, and whole-person health services for older adults
  • especially those living with dementia, complex conditions, and social vulnerability.
This is more than a faculty position. It's an opportunity to build, lead, and teach a better model of aging care. As a clinical leader, you'll drive the development of an Age-Friendly Health System, embedding the nationally recognized Institute for Healthcare Improvement (IHI) 4Ms Framework into everyday care: What Matters

  • Proactive dementia, delirium, and depression care Mobility
  • Preserving independence and function You'll also help design longitudinal, whole-person care models that support not just patients
  • but the caregivers and families who care for them.
What You'll Do Deliver comprehensive, longitudinal care to older adults with complex medical, cognitive, and social needs Serve as a population health leader, improving outcomes for high-risk aging populations Build and advance an Age-Friendly Health System, integrating the IHI 4Ms into clinical practice and education Lead innovative care models for patients living with dementia and cognitive decline Partner with and support caregivers as essential members of the care team Teach and mentor fellows, residents, and interprofessional learners Serve as core faculty in the geriatric fellowship program with opportunities for leadership Collaborate across disciplines in a fully integrated, team-based care model Contribute to quality improvement, curriculum design, and scholarly work Why The Wright Center Mission in
