Job Summary:
The Faculty Clinical Pharmacist works as part of a multidisciplinary team in the Family Medicine Residency Clinic. The Pharmacist will provide clinical pharmaceutical services to patients of all ages and participate in Resident education; as well as provide pharmacologic counseling and education to patients to include diabetes treatment, coumadin dosing, and other chronic disease management.Essential Functions & Responsibilities:
Provides pharmacologic counseling and education to patients, on-site consultations with faculty/Residents, and provides/receives warm hand-offs to/from other faculty and Residents. Obtains medication history from patient and/or their designee. Develops therapeutic plan for diabetes management, anticoagulation therapy, and other chronic diseases. Monitors patients prospectively for any deviation from an accepted treatment plan, for the efficacy of the plan, and for adverse drug reaction and drug interactions occurring during the execution of the plan. Anticipates and recognizes patient problems that are a result of, or will influence, a patient's drug therapy and facilitates solutions when problems occur. Works with faculty and Residents on transition of care clinics to prevent rehospitalization, consults on cases of polypharmacy, and makes recommendations for improving medication regimens for patients, as able. Provides didactic teaching for residents on various pharmacy topics of interest within the Family Medicine context and creates didactic curriculum. Maintains compliance with all state and federal laws and regulations as they pertain to position including HIPAA, sexual harassment, Mandated Reporting, Scope of Practice, OSHA, etc. Adheres to infection control procedures, including but not limited to, standard precautions of temperature monitoring, hand washing, symptom self-monitoring, masking, and social distancing. Performs other duties as assigned.Qualifications:
Doctor of Pharmacy degree from an accredited institution required. Pharmacist license in the state of VA required. Comprehensive computer knowledge and skills of Windows/Office Suite (Word, Excel, Outlook) required. Completion of a PGY1 ASHP- accredited residency (ambulatory care or equivalent practice experience) preferred. Board-certified pharmacotherapy Specialist (BCPS) or Board-certified Ambulatory Care Pharmacist (BCAP) preferred. Didactic and experiential teaching experience preferred. Prior EPIC (EHR) utilization preferred. Commitment to the provision of services for the underserved and a sensitivity in working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic, and cultural origins, disabled, homeless, substance users, HIV (AIDS) infected, and/or physiologically impaired preferred. Required Physical Requirements:
Constant (67-100% of workday) use of arms and hands; frequent (34-66% of workday) standing, walking, and sitting; occasional (0-33% of workday) bending, stooping, and squatting; ability to lift 35 lbs.; ability to push and pull up to 20 lbs.; auditory and visual skills to include color determination.Mental Requirements:
Possesses critical thinking and analytical skills. Ability to multi-task. Ability to communicate effectively and collaborate with a multi-disciplinary team. Capacity to cope with difficult situations. Ability to tolerate irregular hours including evenings, nights, and weekends.Environmental Requirements and Exposure Hazards:
Potential risk of exposure to radiation and toxic chemicals. Potential for exposure to bloodborne pathogens; must be able to wear appropriate personal protective equipment.
