Community Face:
Complete daily rounds in designated hospitals to build lasting relationships with social workers, physicians, discharge planners, and healthcare administrators.
Assess & Facilitate:
Evaluate high-acuity inquiries and assess patient care needs prior to admission, ensuring a seamless, stress-free transition into our center. Collaborate for
Success:
Act as the vital link between physicians, referral sources, and our internal clinical teams to communicate discharge plans and achieve census goals.
Drive Growth:
Educate local civic organizations, home health agencies, and families on our specialized services to meet the community's healthcare needs.
What We Are Looking For:
The Ultimate Connector:
Proven experience in healthcare sales, marketing, or senior living admissions, with strong presentation and relationship-building skills.
Established Network:
Pre-existing relationships with local social workers, discharge planners, and hospital staff is a major plus!
Local Knowledge:
Familiarity with regional hospitals and healthcare organizations is preferred— but we are absolutely willing to train the right person.
