  • Chatham, MA Schedule:
    Monday to Friday, 8am
  • 4pm, in-person
  • no remote work Pay range: $80,000
  • $100,000 per year, based on experience.
Liberty Commons Rehabilitation & Skilled Care is seeking a highly organized and driven Director of Admissions to oversee all aspects of the admissions process in our fast-paced, high-performing skilled nursing facility. This is a key operational leadership role focused on referral management, intake coordination, and ensuring efficient, compliant admissions from initial inquiry through resident arrival. Position Summary The Director of Admissions is responsible for managing the full admissions workflow, including referral intake, tracking, and coordination for both short-term rehabilitation and long-term care placements. This role works closely with our in-house Clinical Liaison and interdisciplinary team to ensure timely, appropriate, and well-coordinated admissions. Key Responsibilities Oversee all daily admissions operations from referral through admission Manage and track incoming referrals for both short-term rehab and long-term care, ensuring timely review and follow-up Coordinate closely with the Clinical Liaison on hospital referrals and screenings Maintain accurate referral and admission tracking systems Utilize PointClickCare (PCC) for referral management, documentation, and census tracking Facilitate communication between admissions, nursing, therapy, and leadership teams Coordinate and conduct facility tours for prospective residents and families Ensure all required documentation, authorizations, and payer sources are verified prior to admission Monitor daily census activity and provide regular updates to leadership Maintain compliance with all regulatory and facility admission policies Qualifications Bachelor's degree required Long-term care (LTC) experience preferred Experience in admissions, intake, or coordination within a healthcare setting strongly preferred Strong organizational skills with the ability to manage high referral volume across multiple payer sources and care types Experience with PointClickCare (PCC) or similar EMR strongly preferred Excellent communication and interpersonal skills Ability to work independently while collaborating with multiple departments Why Join Liberty Commons? 5-Star CMS Rated Facility CARF Accredited Strong clinical team with in-house therapy and specialty programs Supportive, team-oriented leadership environment Opportunity to play a key role in maintaining strong census and operations Compensation & Benefits Competitive salary (based on experience) Full benefits package Paid time off Director of Admissions 4.1 4.1 out of 5 stars 390 Orleans Rd, North Chatham, MA 02650 $80,000
