Job Description
Overview We are seeking a compassionate and dedicated Case Manager to join our team. The ideal candidate will have a strong background in managing children with behavioral issues and intellectual developmental disabilities. This role is essential in providing support and guidance to residents, ensuring their needs are met while promoting their independence and well-being. Responsibilities Develop and implement individualized care plans for residents, focusing on their unique needs and preferences. Conduct regular assessments of residents' health status.. Collaborate with healthcare professionals, families, and other caregivers to coordinate comprehensive care. Maintain accurate records using EMR systems to track resident progress and care interventions. Engage residents in social activities to enhance their quality of life and promote social interaction. Utilize behavior management techniques when necessary to support residents with challenging behaviors. To assist in the preparation of quarterly reports. Schedule appointments for residents and ability to drive residents to appointments. Maintain resident files. Participate in the staffing of each resident's Individual Service Plan. To perform other such duties and accept other such responsibilities as required by the Administrator. Strong writing and communication skills.
