Job Description
We are seeking qualified Social Workers to join our Adult Medical Day Care program in Little Ferry, New Jersey. The Social Worker will provide social work services to adult day care participants, including assessments, counseling, care planning, family communication, community resource coordination, discharge planning, documentation, and participation in interdisciplinary care coordination. The ideal candidate should have experience working with older adults, adults with disabilities, Medicaid/Medicare populations, long-term care, adult day care, senior services, case management, or healthcare/social service programs. This position must meet applicable New Jersey Department of Health requirements for Adult Day Health Services facilities.
Qualifications Required:
Must be currently certified or licensed by the New Jersey State Board of Social Work Examiners as a CSW, LSW, or LCSW Bachelor's or Master's degree in Social Work Experience or strong interest in working with older adults, adults with disabilities, and/or medically complex adults Ability to complete social work assessments, care planning documentation, progress notes, and service coordination records Strong communication skills with participants, families, caregivers, staff, and outside providers Ability to maintain confidentiality and comply with HIPAA and facility policies Must be able to complete required background check, reference checks, pre-employment physical, TB testing, orientation, and required facility training Preferred:
Experience in adult day care, senior services, long-term care, assisted living, nursing facility, home care, hospital discharge planning, case management, or community-based services Experience with Medicaid, Medicare, MLTSS, community resources, caregiver support, and discharge planning Bilingual Korean/English preferred, but not required Responsibilities The Social Worker will be responsible for duties including, but not limited to: Complete social work assessments and psychosocial assessments for participants upon admission and as required Interview participants, families, caregivers, and representatives to evaluate social, emotional, behavioral, family, community resource, and discharge planning needs Participate in the development, implementation, review, and revision of each participant's individualized plan of care Provide or arrange individual, family, or group counseling, supportive counseling, emotional support, and crisis intervention as needed Assist participants and families with community resources, public benefits, housing resources, caregiver support, transportation concerns, medical appointments, and other service needs Coordinate with families, caregivers, physicians, case managers, managed care organizations, community providers, and the interdisciplinary care team Participate in interdisciplinary team meetings, care conferences, case conferences, discharge planning meetings, and quality improvement activities Monitor participant adjustment to the adult day care program and report significant changes in mood, behavior, family circumstances, social functioning, or care needs Assist with discharge planning and coordinate referrals or follow-up services when a participant is discharged, transferred, hospitalized, or requires a change in services Advocate for participant rights, dignity, independence, cultural needs, language needs, privacy, and personal preferences Report suspected abuse, neglect, exploitation, safety concerns, unusual incidents, or significant changes in participant condition according to facility policy and applicable reporting requirements Provide or assist with staff in-service training regarding participant and caregiver psychosocial needs, as requested by the facility Maintain accurate, timely, complete, and confidential documentation, including assessments, progress notes, care plan updates, treatment plans, evaluations, referrals, family contacts, service coordination notes, clinical notes, and discharge notes Cooperate with NJDOH surveys, inspections, audits, quality reviews, and requests for documentation related to social work services Pay:
