Community Liaison / Intake Coordinator (Part-Time) Treasured Gems Home Care Treasured Gems Home Care is seeking an outgoing, relationship-focused Community Liaison / Intake Coordinator to help grow our presence throughout Mobile and Baldwin County, Alabama. This position is ideal for someone who enjoys networking, building referral relationships, meeting with families, conducting assessments, and helping seniors access quality care in their homes.
What You'll Do:
Build and maintain relationships with hospitals, rehab facilities, assisted living communities, skilled nursing facilities, physician offices, hospice agencies, and other referral sources Conduct in-home assessments Follow up on inquiries and referrals Assist families in navigating home care options Coordinate client admissions and service start-ups Represent Treasured Gems Home Care at community and networking events Maintain referral and lead tracking documentation Work closely with office staff to ensure smooth transitions from inquiry to admission
Compensation:
Base Pay:
$1,500 per month Mileage reimbursement Admission bonuses 60-day client retention bonuses Referral source development bonuses
Bonus Opportunities:
Client admission bonuses based on authorized hours Additional bonuses when clients remain on service for 60 days Bonuses for developing referral sources that generate new clients
Qualifications:
Healthcare marketing, home care, hospice, home health, assisted living, senior living, or related experience preferred Strong communication and relationship-building skills Professional appearance and demeanor Reliable transportation and valid driver's license Ability to work independently and manage schedule effectively Experience conducting assessments or admissions is a plus Why Join Treasured Gems Home Care?
