Job Description

Job Summary About This Role Seeking a warm, experienced, and dependable female Team Coordinator to Coordinate Shelby's support team. This person will play a dual role: providing some hands-on care and daily support alongside Shelby and helping to coordinate her small team through scheduling and routine planning. This is not a supervisory role in a traditional sense, but it is about being a reliable, thoughtful presence who helps keep things running smoothly and supports a vibrant, active young woman in living her best life. Shelby is supported by a team of 4 DSP's who all work part time. Shelby is a 27-year-old woman with intellectual and physical disabilities who loves getting out into the community, staying active, and, most importantly, loves music. She has a full life and a full schedule, and she needs someone who can keep up with her energy, anticipate her needs, and bring genuine joy to the work every single day. Who Shelby is Looking For The right person for this role will check all of these boxes — not as a formality, but because they genuinely reflect what this job requires and what Shelby deserves. Is a woman over the age of 21 (Shelby likes to go to live music which is sometimes over 21) Experience working with individuals with intellectual and/or developmental disabilities (preferred) Personal care experience (including. toileting, bathing, and dressing) Genuinely kind — not just polite, but warm and person-centered in how you show up Flexible by nature — you adapt without drama when plans change A real love of music in some form; willingness to engage with it as part of daily life Completely comfortable around dogs — Shelby's family has two large and loving golden retrievers in the home and a love for animals and dogs is non-negotiable since they are family Strong communication skills, both with Shelby and with team members (basic ASL is a plus but not required - we can train on simple signs since Shelby is hard of hearing) Reliable, punctual, consistent, and a self-starter — Shelby counts on her people A sense of humor and the ability to bring lightness to the work - likes to sing a plus Valid California driver's license and clear driving record required — comfort driving throughout the Sacramento/Elk Grove region What You Will Do 1. Assist with Team Coordination Co-develop and manage the weekly schedule for support team members, including daily activity assignments Communicate clearly with team members about shifts, coverage, and any changes to the routine and questions that arise about Shelby Help orient newer team members to Shelby's preferences, routines, and communication style Flag patterns, concerns, or scheduling gaps to the family in a timely and proactive way Maintain organized notes and documentation related to routines, activities, and observations 2. Hands-On Support Provide direct personal care support as needed, including assistance with daily living activities Accompany Shelby on daily community outings — errands, appointments, recreational activities, and social events Physically assist Shelby with walking to ensure balance Facilitate engagement with music-related activities, events, and experiences (this is a big one!) Accompany Shelby into the warm water pool Support Shelby in maintaining her daily routine in a way that is consistent, predictable, and fun Respond calmly and competently to unexpected situations or changes in plans 3. Community & Daily Life Actively support community participation — this is not a sit-at-home job Help identify new activities, experiences, or events that match Shelby's interests and energy Build and maintain positive relationships with community members, venues, and activity partners Champion Shelby's dignity, autonomy, and inclusion in every setting Schedule This is up to a 40-hour-per-week position. Fridays and Saturdays from 8:30 AM to 6:00 PM are required. The remaining hours are flexible and can be arranged around your availability and the team's needs. We are looking for someone who is genuinely flexible and understands that schedules in care work sometimes shift. Nice to Have but Not a Deal Breaker CPR/First Aid certification (or willingness to obtain) Experience creating or managing staff schedules Background in education, occupational therapy, social rec, social work, or related experience Experience with AAC devices or alternative communication systems More About Shelby Shelby is non-speaking and communicates through her own expressive style — AAC device, gestures, facial expressions, sounds, and established routines. Getting to know how she communicates is one of the most rewarding parts of this work, and team members are expected to be attentive, patient, and genuinely curious about learning her language and supporting her communication. Shelby receives the majority of her nutrition through a gastrostomy tube (G-tube). Full training on G-tube feeding will be provided by the family, and no prior experience is required. What is required is a willingness to learn, follow the protocol carefully, and treat this as a routine and dignified part of Shelby's day. Physical Requirements This role involves active, physical work in the community. Candidates must be physically able to: Push a transport wheelchair across a variety of surfaces and terrain Lift a transport chair in and out of the back of a vehicle (approximately 20 pounds) Maintain the physical stamina required for daily outings and active community participation Drive comfortably throughout the Sacramento and Elk Grove region Transportation Shelby has a designated vehicle that is typically used for community outings and transportation. Team members are welcome to use this vehicle. If you prefer to use your own car, mileage reimbursement is provided. A valid California driver's license, a clear driving record, and comfort navigating the Sacramento/Elk Grove area are all required. What Makes This Job Special Shelby is well known in the community. She has a fun, quirky personality, a social life, and strong opinions about how she wants to spend her time. This role will take you to all kinds of places — concerts, community events, shops, restaurants, parks, and more. You won't always be doing the same thing every day, and you will genuinely matter to someone's quality of life, however Shelby does prefer a routine. If you are looking for work that is meaningful, active, and relational, this is it. Shelby is truly a gift to the universe and deserves people who believe the same. She is incredibly loving and funny and bright. We have been told that our family is great to work with and the environment we provide for Shelby is welcoming, kind, and low stress.
