Job Description:
We are seeking an experienced Nurse Practitioner or Physician Assistant with a strong background in advanced wound care and home health to provide high-quality, patient-centered wound care services in patients' homes. This role is ideal for a clinician who values autonomy, flexibility, and delivering specialized care in a one-on-one setting. The position requires availability one day per week and focuses exclusively on advanced wound assessment, treatment, and follow-up in the home environment.
Key Responsibilities:
Perform comprehensive wound assessments in the patient's home Develop, implement, and adjust individualized wound care treatment plans Provide advanced wound care treatments, including but not limited to: Debridement (as within scope and competency) Management of chronic, surgical, pressure, diabetic, and vascular wounds Application of advanced dressings and wound care products Educate patients and caregivers on: Wound care management Infection prevention Treatment plans and expectations Monitor wound healing progress and identify complications requiring escalation of care Communicate effectively with referring providers and interdisciplinary care teams as needed Accurately document all patient encounters using the EMR system in a timely manner Maintain strict HIPAA compliance and patient confidentiality at all times Follow all clinical protocols, regulatory requirements, and best practice guidelines Maintain required licensure, certifications, and clinical competencies
Qualifications:
Active and unrestricted Nurse Practitioner or Physician Assistant license Experience in advanced wound care (required) Prior home health experience (strongly preferred) Strong clinical assessment and critical-thinking skills Comfortable working independently in patients' homes Proficient with EMR documentation Excellent communication and patient education skills Reliable transportation and ability to travel locally
Schedule & Compensation:
Once per week (flexible scheduling within that day) with the opportunity to have more hours.
