$23-$29 Job Description SUMMARY Under the direction of the Program Manager-Director, the Lead Care Manager (LCM)/Resource Specialist provides direct services and coordinates care management activities as part of the Enhanced Care Management (ECM), Prevention, and Aftercare programs. The LCM/Resource Specialist functions as a member of a multidisciplinary Care Team, overseeing assigned cases, coordinating health care and supportive service benefits, providing education, and facilitating timely and cost-effective access to care and community resources. The LCM/Resource Specialist collaborates closely with clients, caregivers, family support persons, service providers, and community partners to promote wellness, recovery, independence, resilience, and long-term stability. Responsibilities include care coordination, service navigation, prevention and early intervention support, aftercare follow-up, resource linkage, advocacy, and monitoring client progress to ensure continuity of care and successful outcomes. The LCM/Resource Specialist serves as an advocate for clients, an active member of the interdisciplinary team, and a liaison between internal programs and external health, behavioral health, and social service providers. This position is responsible for maximizing client benefits, reducing barriers to services, supporting client engagement and retention, and exemplifying the agency's core values while adhering to its mission and vision. To perform effectively in this position, the Lead Care Manager (LCM)/Resource Specialist must: Assess client needs in the areas of physical health, mental health, substance use disorders (SUD), oral health, trauma-informed care, social supports, housing, employment/vocational services, wellness, safety, and community-based resources. Conduct comprehensive screenings and assessments to identify risk factors, service needs, barriers to care, and eligibility for prevention, aftercare, and Enhanced Care Management (ECM) services. Develop, implement, monitor, and update Individual Care Plans (ICP) and Health Action Plans in collaboration with clients, family members, and service providers. Provide prevention and early intervention services designed to reduce risk factors, strengthen protective factors, and promote individual and family well-being. Deliver aftercare services that support long-term stability, recovery, and successful transitions following completion of treatment or other intensive services. Conduct outreach and engagement activities, including phone calls, mailed correspondence, community presentations, home visits, and field-based contacts to connect and retain participants in services. Offer services in locations that are accessible and convenient to clients, including office-based, telehealth, home-based, school-based, community-based, and field-based settings. Connect clients and families to social services, support groups, educational opportunities, health care providers, housing resources, financial assistance programs, and other community supports. Coordinate and advocate on behalf of clients with healthcare providers, schools, social service agencies, legal systems, and other community partners, as appropriate and with client consent. Utilize evidence-based and best-practice approaches, including Motivational Interviewing, Harm Reduction, Strengths-Based Case Management, Trauma-Informed Care, and Client-Centered Engagement strategies. Collaborate with hospitals, treatment providers, and community agencies to support transitional care planning and continuity of services. Accompany clients to appointments, meetings, court hearings, school conferences, or other service-related activities when necessary and appropriate. Provide education on health promotion, mental health awareness, substance use prevention, parenting skills, healthy relationships, safety planning, and available community resources. Assist clients with arranging transportation and overcoming barriers to accessing services. Monitor client progress, evaluate outcomes, update goals, and ensure services remain responsive to client needs. Maintain accurate, timely, and compliant documentation, including assessments, care plans, progress notes, referrals, follow-up activities, and outcome reporting within established timeframes. Participate in multidisciplinary team meetings, case conferences, supervision, and program planning activities. Attend required trainings, certifications, and professional development opportunities to maintain competency and compliance with program requirements. Develop and maintain collaborative relationships with community stakeholders, service providers, and referral partners to enhance service coordination and resource accessibility. Advocate for client self-determination, empowerment, resilience, and equitable access to services while promoting client engagement and retention. Adhere to all agency policies, confidentiality standards, contractual requirements, and regulatory guidelines while exemplifying the agency's mission, vision, and core values. Complete all documentation within the timeframes established by the individual action plans Attend weekly staff/team meetings and supervision. Attend training as assigned (e.g., ACEs Certification).
POSITION REQUIREMENTS
To perform effectively in this position, the individual must have: Interpersonal savvy, demonstrated by the ability to interact with and influence people to establish trust and build strong relationships Ability to do home visits and outreach Experience with usage of EMR/EHRs/Care Management software Strong organization skills and ability to manage and maintain a personal schedule Ability to establish priorities and meet deadlines Ability to work independently within a virtual operating environment and as part of a team Excellent oral and written communication skills Ability to exercise judgment in the application of professional services Active listening skills and genuine compassion for others Enjoys collaboration within a team environment and working with people of different skills and experience Knowledge of community resources in the local market Able to maintain clear professional boundaries with patients and coworkers Commitment to represent the company with professionalism Demonstrates cultural competency and ability to work with diverse groups of community clients Comfortable using computer for documentation, communication, and organizing work
Skill Requirements:
Ability to provide services to various ethnic/cultural groups in the community. Excellent verbal and written communication skills Experience managing crisis and provide crisis intervention. Organized and able to meet documentation deadlines as required by funding contracts.
